DEFINITION:
Perineal care is the care of the perineum
which involves the external genitalia &
perineum to prevent bacterial growth,
which can rapidly increase in a warm ,dark
and moist enviornment.since there are
many orifices(urinary meatus,vaginal
orifice & the anus) situated in this area.the
pathogenic organism can enter into the
body. Thorough cleanliness is essential to
prevent bad odour & to promote comfort.

PURPOSES:
•To promote cleanliness, prevent skin irritations.
•To prevent infection.
•To remove irritating secretions.
•To promote healing.
•To provide comfort.
•To avoid growth of micro organism.
•To prevent bad odour.
•To treat genitourinary tract infections.
•To relieve incontinence.
•To provide post partum care.
•To perform perineal surgery.

INDICATIONS:
•Patient’s who are unable to do self care.
•Patients with genito urinary tract
infections.
•Patient with incontinence of urine and
stool.
•Patient with excessive vaginal discharge or
bleeding.
•Patient with indwelling catheter.
•Post partum patients.
•Patient with injury, ulcer or surgery to the
perineal area or rectum.

PRELIMENARY ASSESSMENT:
•Assess the condition of perineal skin any itching,
irritation, ulcers,odema, drainage etc.
•Assess the need and frequency of perineal care.
• Assess whether the perineal care should be done
under aseptic techniques or a clean technique.
• Check the physicians order for any specific
instructions.
•Assess the patients ability for self care.
•Assess the patients mental state to follow instructions.
•Check for the articles available in patients unit.

• Tray containing mackintosh (big)
• Towel
• Wet cotton balls in a bowl
• Gauze piece
• Artery forceps
• Pair of gloves
• Kidney tray
• Paper bag
• Pads
• Drape sheet
• Bedpan
• Screen
• Perineal set
• Cotton pads.
• Distil water.
• Betadine lotion/ointment.

PREPARATIONS OF PATIENTS AND UNIT:
• Explain the procedure to the patient
• Provide privacy by screen & drapes. Drape the patient for
vaginal examinations. Fold the top linen to the foot end of
the bed.
• Remove all articles that may interfere with the procedure.
E.g. aircushion, knee pillows etc.
• Give extra pillow to raise the head.
• Roll the draw sheet to the opposite side to prevent soiling
when bad pan is placed or place a mackintosh under the
buttocks, over the draw sheet.
• Offer the bedpan. Keep the clean bedpan on the bed on
your working side. Ask the patient to flex the knees & lift
her buttocks by pressing the foot against the mattress.

• Place the bedpan is position while the nurse
slides her left aim under the waist & lower the
patient gently over the bedpan. Adjust it
comfortably for the patient (use plastic gloves for
this).
• Untie the pad, if any and observe the discharge,
its color, odour amount etc.discard the pads in
the paper bag or the segregation bucket.
• Leave the patient for sometime so that she can
pass urine or stool if necessary.
• Remove the bedpan .discard the plastic gloves &
wear a sterile pair of gloves.
• Before putting in sterile gloves arrange all the
necessary articles near to the patient.
