II. PROJECT RATIONALE:

The Local Government Unit of Padada is of the municipalities in the province of

Davao del Sur. It has 17 barangays under its jurisdiction with projected population of
26,507, It has 154 Barangay Health Workers and 83 active CHT. Our annual IRA for this year
is more or less fifty-five million pesos with limited budget allocated for health; the LGU
allocated 280,000.00 budgets for medicines, supplies and equipments. Considering the
number of clients to be served, such amount is too small. Increase of funds is definitely
restricted because there are other priority programs. Thus, the idea of tapping external
assistance came up for this purpose.

III. PROJECT OBJECTIVE:

The main objective of this project is to improve quality of life through quality health
services and humanity care.

IV. PROJECT SUSTAINABILITY:

Provision of small fee to cater the maintenance of the ambulance so that the
services on emergency transport will be sustained for a long time. The ambulance driver will
maintain logbook and write periodic progress report.

V. DESIRED OUTCOME OF THE PROJECT:

To enhance Maternal and Child Health Status

Provide a fast mobility ambulance sources in case of emergency and extreme
sickness
Reduce incidence in loss of life in relation to pregnancy and giving birth
Increase number of institutional deliveries

The project will help the constituent of Padada to improve their health conditions by
providing better access to quality health services. This is also in line with the Millennium
Development Goals of the National Government.

V. PROJECT ADMINISTRATION AND MANAGEMENT:

The responsible office that will administer the proposed project will be the Municipal
Health Office.

VI. RESPONSIBILITIES OF THE DOH:

Provide training in the operation, repair and maintenance of the equipments.

Conduct monitoring and evaluation of the project.

VII. RESPONSIBILITIES OF THE LGU:

Allocate budget for the replacement and maintenance of the equipments.

Coordinate with DOH as to the implementation of health programs
Conduct regular monitoring and evaluation
