RESOLUTION NO. 2017 - 121

RESOLUTION RESPECTFULLY REQUESTING HON. PAULYN

JEAN B. ROSELL-UBIAL, SECRETARY OF THE DEPARTMENT
OF HEALTH (DOH), TO DONATE ONE (1) UNIT OF
AMBULANCE FOR THE USE OF WEST ISULAN BIRTHING
CLINIC LOCATED AT LAGUILAYAN, ISULAN, SULTAN
KUDARAT
WHEREAS, the West Isulan Birthing Clinic located at Isulan, Sultan Kudarat has been
delivering free, accessible and emergency health care services to indigent patients in the
municipality of Isulan, Province of Sultan Kudarat as well as patients of nearby localities of
Esperanza and Bagumbayan, Sultan Kudarat;

WHEREAS, the birthing clinic created the position of ambulance driver in anticipation
that the provincial government of Sultan Kudarat could provide one (1) unit ambulance to be
used in serving the people in emergency cases;

WHEREAS, considering the incapacity of the provincial government to provide said

request, there is a need to request the Department of Health for one (1) unit of Ambulance to
serve and address the needs of the sick with immediate resolute and efficiency;
 WHEREAS, after a thorough deliberation, the members of this Honorable Body are in
accord to request the Department of Health to donate one (1) unit of ambulance for the use of
West Isulan Birthing Clinic;

WHEREFORE, upon motion of Hon. Cyrus E. Torrea and duly seconded by

Hon. Jose Remos P. Segura, it was-

RESOLVED, as it is hereby resolved, to respectfully request Hon. Paulyn Jean B.

Rosell-Ubial, Secretary of the Department of Health, to donate one (1) unit of ambulance for the
use of West Isulan Birthing Clinic located at Isulan, Sultan Kudarat;

RESOLVED FURTHER, to forward copies of this resolution to Hon. Paulyn Jean B.

Rosell-Ubial, Secretary of the Department of Health, Manila, for her information, guidance and
appropriate action.
