repair of 10 units Musical Instruments. The said amount is Forty Thousand Pesos Only
(Php40, 000.00) under our approved budget Activity No. 5 Goal no. 2 Objective C Measure 0.
The stipulated amount is from our budget of Repairs and Maintenance of Instruments. The full
amount is payable to Mr. Felix P. Velez.

Attached herewith is the quotation of instruments to be repaired.
