AFFIDAVIT OF WITNESS

I, DIVINO MIAG-AO, Filino, of legal age, married and a resident of 123

Road 1, Mabuhay Village, Dao District, Tagbilaran City, Bohol, Philippines after
having been duly sworn to in accordance with law do hereby depose and state that:

1. On July 4, 2017 at about 11:30 in the evening, I was sleeping in our

bedroom at our residence located at 123 Road 1, Mabuhay Village, Dao
District, Tagbilaran City;

2. I was awoken when I suddenly felt an excruciating pain on my groin area;

3. I saw my wife Ellena Miag-ao y Adarna in front of me holding a butchers

knife on her right hand while her other hand was bloodied and was holding
my mutilated sexual organ;

4. I cried loudly in extreme pain and asked for help from our house help when
I saw her standing outside our bedroom;

5. I am deprived of my right to reproduce offsprings or to transmit life due to

mutilation of my sexual organ;

6. I am executing this affidavit of my own free will and volition that nobody
forcing me to do so and to file a criminal complaint against accused Ellena
Miag-ao y Adarna.
