Dear Parents,

My name is Leanne Whyte. I am a Pre-Service teacher with Charles Darwin

University, undertaking my final placement in your childs class for the next 10
weeks. I am looking forward to spending time at Larapinta and getting to know
the children and families of 3/4 Morton.

As part of the requirements for my final placement I am required to compile an

electronic portfolio. This will include de-identified: photographs and work
samples, video of lessons and written reports about my teaching and planning. I
am requesting your permission to include your childs de-identified photographs
and work samples in my portfolio. If you do not give permission for me to use
these please sign the form below and return it to school with your child.
