CHORIOCARCINOMA

Definition
Aggressive malignant tumor ,arises either from gestational chorionic
epithelium, or less commonly ,from totipotent cells within the
gonads(as a germ cell tumor)

OR
Malignant neoplasm composed of large sheets of biphasic markedly
anaplastic trophoblast without chorionic villi
TYPES
MORPHOLOGY
Soft, fleshy, yellow white tumor
Hemorrhagic necrotic uterine masses
Primary lesion may self destruct
Metastasize to myometrium and vessels
Chorionic villi absent
Tumor composed of anaplastic
cuboidal cytotrophoblast and
syncytiotrophoblast.
Clinical Features
Manifests as bloody , brownish discharge .
Hematogenous spread to lungs (50%) , vagina (30-40%) ,brain, liver,
kidneys.
Lymphatic drainage is uncommon.
100% cure by chemotherapy in gestational choriocarcinomas
Poor prognosis of germ cell choriocarcinomas (lack of paternal
antigens on gonadal tissues)
Diagnosed by high B hCG titers in blood and urine.
