Clinical scenario

You are a 2nd year medical student on your GP placement. Mr Ely Levitt is a 60 year old man who has recently been
diagnosed with congestive cardiac failure. He has returned for a review a month later. Your supervising GP, Dr smith,
has started to examine him and found no signs of clubbing or cyanosis.

Pulse rate = 70 bpm

Respiratory rate = 16 beats/min

Blood pressure = 140/90mg

Dr Smith has asked you to complete Mr Levitts examination to assess his congestive cardiac failure.

- In a total of five minutes:


o Complete his congestive heart failure assessment
o Tell the patient what you are doing as you examine him
- You do NOT need to take any further history

Look for:

- Start with inspection; end-of-bed-o-gram


- Peripheral edema
- Basal crepitations
- Shifted apex beat
- Palpate for liver
- Examine JVP (bed at 45 degrees)
