NOMINATION DIRECTIONS:
In narrative form, individually address all five nominee bio areas as listed below. Each section
of the bio will be scored equally during the selection process. Only one person may be nominated per form.
The complete nominee bio may not be more than two one-sided 8.5 x 11 pages, printed
or typed, in addition to the nominee/nominator information on the back of this page. Include nominees name
on each page. Do not submit any additional documents other than the two one-sided 8.5 x 11 pages.
Online submission is encouraged.
Do not assume selection committee members know your nominees qualifications.
Multiple nominations for the same person will be reviewed and evaluated separately.
You may want to consult with others, even your nominee, to be sure you are including all of your nominees
activities and accomplishments.
High school student must attend a Kent Intermediate School District school.
