No.42401-1939684-7, do hereby solemnly affirm and undertake as under:

1. That I have purchased the Shop in which Telephone No.021-36724458,

A/c ID No.210766050, has installed at Shop No.F-11, Block-H, 1st Floor, Saima Paari
Mall, North Nazimabad, Karachi, which is in the name of AZIZ AMEER ALI
ALWANI.

2. That the attached application with necessary documents for purpose of names is a part of
this undertaking.

3. That the above mentioned PTCL Telephone Number may be transferred in my name
only. In case of any objection raised from any person / Department, the PTCL will not be
held responsible and PTCL has the rights to cancel transfer of name and disconnect the
supply without any further notice.

4. That after the above mentioned meter in my name I shall undertake that I shall pay all
sorts of dues, supplementary bills, Detection bills, or any kind of bills, dues if work out
against the above mentioned meter for previous period. In case of defaulter the PTCL has
fully right to take action according to PTCL rules.

5. I further declare that I shall abide by rules and regulations of PTCL in this regards if any
objection or claim filed with PTCL and title to the property is found defective in future,
The Company has complete right to revert in the name of previous registered Customer
and I shall have no claim in this regard.

6. That whatever stated above is all true and correct to the best of my knowledge and belief.

name of RAFIA AZMAT.

8. That the attached application with necessary documents for purpose of names is a part of
this undertaking.

9. That the above mentioned meter may be transferred in my name only. In case of any
