AFFIDAVIT
I, LITA G. MONTERDE, of legal age, Filipino, married and a resident of Brgy. New
Balanac, Villaba, Leyte, Philippines, after having been duly sworn to in accordance with law,
do hereby depose and state that:

1. I am an incumbent Barangay Treasurer of Brgy. New Balanac, Villaba, Leyte,

Philippines;

2. During my term of office or service requirement, I was NOT employed in the

government;

3. I DID NOT receive any form of salary/compensation from the government, except
my honorarium for holding a position in an ex-officio capacity as Barangay
Treasurer, during my term of office or service requirement;

5. I am willingly and voluntarily executing this Affidavit to attest to the truth of the
foregoing facts and to support my application for Grant of Eligibility Pursuant to
R.A. 7160 (Barangay Official);

6. This affidavit is also executed for any and whatever legal purpose that this may
serve.
