Re : QUOTATION FOR SECURITY GUARD SERVICES (UNARMED)

We refer to the above and do understand that your company is looking for a well organized and a
reputable security company to take over the security surveillance and safe guard your business
and its immediate vicinity.

As such, we forwarding herewith the details of services as required and we have taken note of all
your requirements of positioning of guards as well as other relevant duties that needs to be carried
out.

1. LOCATION TO BE COVERED

Semi D House at Dengkil premises

2. DEPLOYMENT OF SECURITY PERSON

Day Shift ( 0700hrs 1900hrs ) - 2 Security Guards

Night Shift ( 1900hrs 0700hrs ) - 1 Security Guard

3. SERVICE RATE CHARGED

1 Security Guard : RM 6.50 Per Man Hour

4. COMPUTATION

Day Shift ( 0700hrs 1900hrs )

2 Security Guards x 12hrs x RM 6.50 x 30.5 days = RM 4, 758.00

Night Shift ( 1900hrs 0700hrs )

1 Security Guard x 12hrs x RM 6.50 x 30.5 days = RM 2, 379.00

Sub Total = RM 7, 137.00

6% Govt. Service Tax = RM 428.22

Rounding Adjustment ( - ) = RM 0.02

TOTAL = RM 7, 565.20

5. VALIDITY OF SERVICE RATE

This quotation rate valid for one (01) month due on the date of issued.

6. TERMS OF PAYMENT

Payments shall be made 14 days upon receipts of an official invoice.

7. CONTRACT

A standard contract to be signed by both parties to safe guard the interest of all concerned
upon commencement.

8. TERMINATION OF CONTRACT

One month notice by either party.

9. DATE OF COMMENCEMENT

A minimum two weeks notice is required to enable us to recruit the necessary manpower
to be deployed. However we will ensure that we will put in force at the earliest possible
date.

10. INSURANCE CLAIM

Any claim or loss has to be justified by show of proof on the negligence to the security
personnel involved.

A proper investigation has to be conducted without prejudice. All angles or avenues have
to be exhausted before determining the cause of loss, which have to be agreed by both
parties.

11. SUPERVISION

The security services will be monitored by an Operation Officers on a daily basis, this will
enhance the standard of services provided to the clients.

12. REPORTING PROCEDURE

All correspondence, reports and liaison shall be made to SEBIRO SECURITY

SERVICES SDN BHD.
