AFFIDAVIT OF SERVICE AND FILING

I, ______________________, mailing clerk of _______________________, with office

address at ________________, after being duly sworn, depose and say:

That on September 22, 2017, I filed a copy of the Judicial Affidavit, after
serving a copy thereof through LBC, a private mail courier, to the counsel of the
adverse party in accordance with Section 2, of A.M. No. 12-8-8-SC or the Judicial
Affidavit Rule.

I deposited the said copy of the Judicial Affidavit with LBC in a sealed
enveloped plainly addressed to the addressee named below as evidenced by the
receipt issued by LBC with instructions to LBC to return the mail to sender after ten
(10) days if undelivered.

Service by a licensed courier was resorted because the law office of the
plaintiffs counsel was closed and locked when personal service was attempted on
September 22, 2017. Upon inquiry with the Security Guard of
________________Building and persons in the nearby offices, staff of the plaintiffs
counsel attended a fiesta. Thus, there is no one to receive the copy of the Judicial
Affidavit.

Therefore, I am executing this Affidavit to attest to the truth of the foregoing

and for whatever legal purpose/s this may serve.
