CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF LEASE, made and entered into this _______________at


__________________, by and between:

We/I, ____________________________________ both/of legal age, single/married with


residence and postal address at____________________________________________,
hereinafter referred to as the LESSOR/S,
-and-

____________________________________ also of legal age, single/married with


residence and postal address at____________________________________________ hereinafter
referred to as the LESSEE/S,

WITNESSETH: That

WHEREAS, THE LESSEE/S hereby accepts to lease from the LESSOR/S a


house/apartment located at
________________________________________________________________, subject to the
following terms and conditions to wit:

1. TERM OF CONTRACT - That the terms of this lease shall be _____ (____) years; to
commence on _______________________________ and will expire on
______________________; renewable upon the option of the LESSOR/S and subject to terms
and conditions accepted to both parties;

2. RENTALS- That the LESSEE/S shall pay the LESSOR/S a monthly rental of
_________________________ PESOS (Php __________ .00), Philippine Currency;

a. Upon the execution of this contract of lease, LESSEE/S shall pay the LESSOR/S
an amount equivalent to two (2) months advance and two (2) months deposit or
the total amount of ___________________________ (Php ______________)
upon signing of this contract;

b. That in case the LESSEE/S fails to pay the above monthly rentals, the lease shall
be deemed terminated and the LESSEE/S shall peacefully and voluntarily vacate
leased premises and surrender the same with LESSOR/S without further need of
demand, otherwise, LESSOR/S shall be constrained to eject LESSEE/S from the
leased premises and padlock same terminated/disconnect the water and electric
(Meralco) supply. Any personal property left behind by the LESSEE/S should be
turned over to the Barangay for proper disposition. Should the LESSEE/S leave
behind unpaid rentals and the balance of hi/her deposit after deduction will be
sold by public auction and the proceeds thereof to be applied to the unpaid rentals;

c. That the said monthly rental shall be increase Ten percent (10%) every year;

3. That the premises leased have been received by the LESSEE/S in good, healthy and
habitable condition and shall maintain the same during the term of this lease;

4. That all ordinary repairs within that arise in the daily use of the facilities therein shall
be for the sole expense of the LESSEE/S, without right to reimbursement;
5. That the LESSEE/S shall use the leased premises exclusively for the purpose leased
and not for any other purposes;

6. That the LESSEE/S is expressly prohibited to sublease the leased premises to any third
party and is likewise prohibited to store any inflammable or combustible chemicals as well as
prohibited drugs therein;

8. That all charges for water, light and telephone shall be to the sole account of the
LESSEE/S during the term of the lease;

9. That LESSEE/S shall be responsible for the observance of sanitary and electrical
regulations imposed by the city government for the use and habitations of the leased premises;

10. That the LESSEE/S shall notify the LESSOR/S at least sixty (60) days in advance
should he decide to terminate the lease during the effectivity of this lease contract or prior to the
termination of lease while contract is in effect the deposit and advance shall be forfeited;

12. LESSEE/S shall allow the LESSOR/S or their unauthorized representative to inspect
the leased premises at any reasonable time;

13. Should LESSEE/S fail to pay two (2) consecutive monthly rentals LESSOR/S shall
have the legal right to eject the LESSEE/S from the leased premises without the necessity of a
court order, and in any violation on the terms and conditions set forth in this contract and
LESSEE/S shall immediately vacate the leased premises and surrender peacefully possession
unto LESSOR/S.

BEFORE ME, a Notary public for and in the City of Muntinlupa this

persons who executed the foregoing instrument and they acknowledge to me that the same is
their free act and deed.
This instrument related to a contract of lease consisting of two (2) pages including the
page where this acknowledgement is written and signed by the parties and their instrument
witness at the first page hereof.
