MANIFESTATION

Petitioner JUANA DE LA CRUZ (Petitioner), by counsel and

unto this Honorable Court, most respectfully states:

1. On _____________ 2017, Petitioner filed a Petition for

judicial declaration of nullity of marriage before this Honorable
Court.

2. Under Section 5(4) of A.M. No. 02-11-10-SC1, the

petitioner shall serve a copy of the petition on the Office of the
Solicitor General (OSG) and the Office of the City Prosecutor (OCP),
within five days from the date of its filing and submit to the court
proof of such service within the same period.

3. Petitioner respectfully manifests that on _____________,

the service to the OCP was made personally while that of the OSG
was done via registered mail by the undersigned counsel in view of
the time constraints, the shortage of available manpower to effect
personal filing and service, and the considerable distance between
the office of the undersigned counsel and the address of the OSG.

4. Petitioner further respectfully manifests that the copy of

the Petition furnished the OSG was sent under Registry Receipt No.
782803840.
1 Rule on the Declaration of Absolute Nullity of Void Marriages and Annulment of Voidable
Marriages.
 5. As proof of the above service, Petitioner hereby attaches a
copy of the above Registry Receipt No. as well as the received stamp
with date by the OCP as Annex A.
