the MERALCO Electric Meter Account under my name since we apply for a new meter
last 2001.

Since then, I also certify that Andrian's Parents, Myler L. Floro and Maria Elisa L. Floro,
are in full control and responsibility of the said MERALCO account's electric bill and other
charges.

Herein attached a photocopy of my Identification Card to duly attest and confirm this
certification.
