I humbly request for a registration of this lost RC booklet and/or an affidavit for the RTO so that the RTO
can issue a duplicate RC book to me. If a RC Book Missing challan needs to be issued as part of the
process, please help me understand the process and documentation required.

Please find attached copies of documents as proof of ownership: Purchase Proof (Sales Invoice), Valid
Insurance, Address Proof, PUC, Copy of RC book, Drivers License.
