Obtaining feedback on your performance as a manager can be very useful to enable you
to identify areas where improvements might be required. A 360 appraisal process offers
the opportunity to obtain feedback from many colleagues who work with you rather than
just your own line manager.
It may be the case that your organisation does not operate a 360 appraisal system.
Nevertheless, collecting information about your own performance can be very beneficial
and can help you to build on your strengths as a manager and identify any areas that
need some improvement.
Below is an example of a feedback form that could be adapted (if necessary) to gather
information about your own performance.

As valued colleagues I would be grateful if you would spend a few minutes of your time to complete
this questionnaire which will inform me of your views on my workplace performance as a manager
and leader of my team.
Please be as honest as you like, as I will use your feedback to assess my performance and to help
me to update my Personal Development Plan.
Please respond to each of the statements below, rating my performance as 1 excellent; 2 very good;
3 good; 4 fair; 5 poor.
