I have the honor to request from your good office to transfer from Don Amadeo Perez, Sr.
Memorial Central School-Main to Nancalobasaan Elementary School for the following reasons:

1. That I am presently teaching as Teacher I at Don Amadeo Perez, Sr. Memorial Central
School-Main, San Vicente Central , Urdaneta City and I am residing at Nancalobasaan
Urdaneta City;
2. My travel expenses from Nancalobasaan to San Vicente back to Nancalobasaan after
school hours is P50. My possible transfer to Nancalobasaan Elementary School would
lessen my transportation expenses down from P50 to P 20 a day;
3. My Food expenses during school days is P100. My possible food allowance will lessen
down from P100 to P30 a day;
4. To lessen my travel time for effective time management;

I am hoping this request merits your kind consideration and approval.

3. My Food allowance is P100 per day. It will lessen to P30.00. Guaranteed I will be
transferred to Nancalobasaan Elementary School;
4. To lessen my travel time for effective time management;
