SUBJECT : Spot Report on RIR to Homicide and Damage to Property

DATE : December 31, 2017

1. Please be informed that on December 30, 2017 at around 7:00 PM, a

vehicular incident transpired along Barangay Road (Bugallon Proper/Norte going to San
Miguel, Ramon, Isabela) involving a (Vehicle-1) Toyota Hilux, color super red II, bearing
plate number ABI 4138, with Engine No. 2KDS440407, Chassis No.
MROER39G60769442, registered under the name of Marilyn Reyes y Salum of
Bascaran, Solano, Nueva Vizcaya and being driven by EDGAR REYES y SECOLLES
(arrested) ,48 years old, married, registered civil engineer and a resident of Barangay
Pangal Sur, Echague, Isabela and a (Vehicle-2) RS Yamaha 100, color red, bearing plate
No. BN-6890 and driven by ROLLY SEBASTIAN y ARELLANO (deceased), 40 years
old, married, farmer and a resident of Purok 3 Bugallon Norte, Ramon, Isabela.

2. Initial Investigation disclosed that veh-1 was then traversing west direction
towards Brgy. San Miguel this municipality while the Veh-2 was coming from opposite
direction (east bound), The Veh-2 is then on the process of overtaking and upon reaching
the place of incident both vehicles collided, (Vehicle-1 to its left front bumper while
Vehicle-2 to its front wheel). The driver of the Veh-2 sustained severe injuries and was
immediate brought by the responding Rescue 316 of this Municipality at Southern Isabela
General Hospital, Santiago City for immediate medical treatment but unfortunately he
died while undergoing medical treatment. Both vehicles incurred damages of
undetermined costs and it was brought to this office with the driver of Veh-1 for proper
documentation and disposition.
