JUDICIAL AFFIDAVIT OF JERRY T. CO

That I, Jerry T. Co, of legal age, married and resident of Brgy. Antonino, Alicia
Isabela, after having been duly sworn to an oath depose and declare the following:

That I am being examined by counsel, Atty. Anthony A. Foz, at his law office
located at #7 Rose St. Baptista Village, Calao East, Santiago City, Isabela, fully
aware of my responsibilities for perjury or false testimony;

That I am offering this judicial affidavit as my direct testimony in court to prove

the following:

1. To prove that I am the registered owner of one half (½) portion of the lot
85-A Cad 211 located inPanganiban St. Centro East Santiago City embraced by TCT
No. TSC-35398;

2. To prove that I lost beyond recovery sometime in 2013 the owner’s

duplicate copy of TCT No. TSC-35398;

3. To prove that I have executed an Affidavit of Lost of the said owner’s

duplicate copy an that I have cause the annotation and recording of the affidavit of
lost which I have executed in the file copy of the same title in the Office of Register
of Deeds of Santiago City under Entry No. _____________________ ;

4. To prove finally that my brother Jason T. Co executed a Deed of Re-

conveyance of one half (½) portion of the lot embraced by TCT No. TSC-35398____
in favor of Zaldy Jojo T. Co;
 QUESTIONS AND ANSWER

1. Q. Mr. jerry Co, you said you are the owner of one half ( ½) portion of the lot
embraced by TCT No. _TSC-35398 ?
A. Yes Sir.

2. Q. Where is the owner’s duplicate copy of TCT No. _TSC-35398 ?

A. I have lost it beyond recovery sometime in 2013;

3. Q. What did you do in connection with the lost of your certificate of title?
A. I exerted earnest efforts to find and locate the title but I could not find it
anymore;

4. Q. After exerting efforts to find and locate your certificate of title what did you
do next?
A. I executed an affidavit of lost and caused the affidavit of lost to be
annotated and recorded in the file copy of the same title with the Office of
Register of Deeds of Isabela;

5. Q. What do you want this court to do with the petition?

A. I pray that the petition for issuance of the owner’s duplicate copy of TCT
No. _TSC-35398 be granted so that the Deed of Re-conveyance of one half ( ½)
portion of the lot be registered in favor of Zaldy Jojo T. Co;
6. Q. Do you have anything more to state?
A. None in the meantime sir.
