2. I passed the Chemistry Licensure Exam on September 2012;

3. Until now I have not claimed my Board Certificate from the

Professional Regulation Commission;

4. I am executing this affidavit to attest to the truth of the foregoing

narration of facts for whatever legal purpose this may serve.
