2051 In-vitro Prevention of Incipient Caries Lesions during

Orthodontic Treatment

One of the most common negative side effects of orthodontic treatment with fixed appliances is
development of incipient caries lesions around brackets, particularly in cases with poor oral
hygiene. Bioavailable minerals from ACP-containing cement may locally reduce enamel
demineralization.
Objectives: To investigate the potential of ACP-containing resin cement and other treatments
(fluoride varnish, resin sealer, MI-paste) to prevent incipient caries lesions on bracketed teeth.
Methods: One-hundred extracted human premolars were randomly allocated to five groups
(n=20). Brackets were bonded with ACP-cement (Aegis-Ortho), Transbond XT (TbXT, Control)
or with TbXT followed by application of fluoride varnish (Vanish), resin sealer (Pro-seal), or
CPP-ACP paste (MI-Paste). All groups were cycled for 15 days in demineralization solution for
8 hours/day, rinsed, placed in artificial saliva (30 min), brushed and stored overnight in artificial
saliva. The extent of demineralization in each group was assessed using QLF (Quantitative
Light-induced Fluorescence), and CLSM (Confocal Laser Scanning Microscopy).
Results: Fluorescence loss (ΔF, QLF) and lesion depth (µm, CLSM) were recorded for each
group (Mean±SD).

Pro-seal and Vanish groups had significantly smaller lesions (Kruskal-Wallis; p<.05) than the
control group for both QLF and CLSM.
Conclusions: While Aegis-Ortho and MI-Paste groups showed preventive effects when
compared to Transbond XT control, adjunctive application of Pro-seal and Vanish were the most
effective treatments for prevention of enamel demineralization during an in-vitro acid challenge.
