INSTRUMENTS FOR LAPAROSCOPIC PARTIAL NEPHRECTOMY

Optimal oncologic control of small renal cortical neoplasms while maximally

preserving kidney function has been the goal. Reproducing the open surgical
technique while maintaining a minimally invasive approach has been the
challenge.

Novel minimally invasive technologies including laparoscopic vascular and

kidney clamps and new energy sources now provide the laparoscopic
surgeon the opportunity to reproduce the technique of traditional open
partial nephrectomy. As such, patients experience all the advantages of open
partial nephrectomy with the clear benefits of laparoscopic renal surgery.

Various thermal devices have been used to reduce bleeding during the
excision of the tumor, such as ultrasonic shears, bipolar and monopolar
energy devices, and argon beam coagulation. However, with the application
of energy devices, the renal parenchyma becomes charred resulting in the
inability to discern the margins of the tumor. Additionally, coagulated tissue
becomes fragile, which hampers placing sutures. The tissue response to
surgical energy devices can also result in delayed urine leakage resulting
from liquefaction of the necrotic tissue. As such, alternative techniques to
successfully perform laparoscopic partial nephrectomy have been developed.

Articles published in The Journal of Urology and The Journal of Endourology

by Clayman, Landman, Gill, and others, report the advantages of renal hilar
vascular control. This results in better visualization for improved precision
during tumor excision, to facilitate hemostatic suturing of the parenchymal
defect, to decrease blood loss, and to minimize operative time.

The innovative and unique laparoscopic vascular clamps from Aesculap

include bulldog vessel clips of varying sizes and configurations, and
Satinsky-like laparoscopic vascular clamps with their associated trocars with
flexible cannulas to permit introduction of curved jaw designs.

The Aesculap Simon Renal Pole Clamp is a kidney clamp which can minimize
the concern of ischemia time constraints by allowing continuous blood
perfusion to the functioning kidney during laparoscopic partial nephrectomy.

Aesculap’s vascular sets for advanced urologic surgery have been widely
adopted by the urologic community for laparoscopic partial nephrectomy and
live donor nephrectomy, and are considered by many to be the “Gold
Standard.”

Aesculap laparoscopic instruments such as a right angle dissector, L or J

hook electrodes, macro bipolar graspers, large jawed scissors, axial needle
holders, and many other select designs have become indispensable in the
armamentarium of many urologists for laparoscopic partial nephrectomy and
other laparoscopic procedures.
