Subject: Bushings of the B+V VES-SD Elevators

Purpose: To inform the industry about a near incident

Recommendation: Modify the Bushing for Casing/Drill Collars

Effective: All Casing and Drill Collar Bushings for B+V VES-
SD 500 Frame 1, 2, 3 and VES-SD 750
Description of incident:

Blohm +Voss was informed of an incident using the VES-SD Elevator. The VES-SD elevator was
dressed with a incorrect (too large) bushing for the casing to be run. The VES-SD elevator was
closed around the casing and the weight supported in the elevator. However the coupling of the
casing passed through the upper bore (flange) of the bushing and settled on the lower bore
(flange) of the bushing (see picture #1). The fault was discovered after the entire string load was
suspended for several hours.
No personal was injured nor was there any damage to the equipment. In an effort to prevent
reoccurrence B+V would like to make a recommendation to the industry.

Recommendations to the industry:

1. Check the marking on top of the Bushing and measure the ID of the Bushing and pipe
OD before using the Elevator together with the set of Bushing.

2. Bevel the lower section of Bushing (see Picture 2). This will prevent the possibility of
supporting any load on the lower bore (flange).
