AFFIDAVIT

I SITTI MONA SALAPUDDIN GAMPAL-CAMLIAN of legal age, Filipino citizen,

married and resident of Baliwasan Tabuk, Zamboanga City, Philippines, after having
been duly sworn to in accordance with law, hereby depose and say that:

1. I am the biological mother of HARRIS GAMPAL CAMLIAN;

2. However, my child was issued a travel document because he was born in

Singapore on 12 March 2014 and we have not applied for any passport yet for
we have to come home;

3. He entered the Philippines using Travel Document No.067130, issued by the

Philippine Embassy, Singapore, issued on 25 March 2014 and valid until 24 April
2014;

4. I am executing this Affidavit to attest to the truth of the foregoing facts and to
support his application for a new passport with the DFA Regional Consular office,
Zamboanga City, Philippines;

IN WITNESS WHEREOF, I have hereunto set my signature this 8th May 2014 at
