AMENDMENT PROCEDURE
Proposals for amendments to documents should be formally submitted to the Document Custodian identified
on the front page of the document by using an Operating Procedure Document Control Form.
An Operating Procedure Document Control Form is available on “The Well” via the document link on
Santos Corporate EHS Management Standard : EHSMS11.08 – Operating Procedures and Safe Work
Practices- Appendix E or via Indonesia Operations Team Site.
If this form cannot be accessed via the intranet, personnel should contract their immediate supervisor for
help on submitting the Operating Procedure Document Control Form. All requests must be made via
Operating Procedure Document Control Form, and a separate form is required for each change requested.
In agreeing to any changes to the document, the Document Custodian will assign a priority rating to the
request which will directly relate to the urgency of the request. The Document Custodian shall then instigate
the approval process.
Once approved, the Document Custodian must ensure the amendment is incorporated into the relative
EHSMS document on the Santos Indonesia Operations server.

1. PROCEDURE AND INFORMATION

PROSEDUR DAN INFORMASI
A. SAFETY AND ENVIRONMENT PRECAUTION

1. Operation will be carried out in accordance with FSO Operation Manual SHAN-0320
2. JRA will be prepared.
3. Toolbox meeting with all parties involved in operations.
4. Deploy fenders on Port Side.
5. No oil spillage into the sea
6. All operation to be carried out day light only.
7. All operation to be carried out weather permitting.
8. Customs to be informed and permission to be obtained for breaking Export valve E4
seal.
B. FLUSHING HOSE
1. Support vessel to disconnect Export Hose from Sinker buoy and bring to Shanghai Port
Side manifold.
2. Export Hose to be connected to Port manifold and flushed with water.
3. Flushing to be carried out with sea water until a clean sample from the Port manifold
sample cock received (the hose volume must be displaced at least 3 times).
4. After flushing with water P Manifold Valve to be closed and from Aft Manifold a
compressed air to be introduced.
5. Pressure to be raised to 5.0kg/cm² and subsequently P Manifold Valve to be open
allowing the fluid to be displaced by the air. Stop air supply by shutting the air valves.
6. Keep open P manifold valve until air start passing trough the P/S manifold valve. Keep
open the manifold sample cock during the air flushing. When air start coming out from
the manifold sample cock shut the P/S manifold valve. Keep open the manifold sample
cock till the hose is depressurized.
7. Shut Aft Manifold Valve.
8. Shut and lock the Hose valve.
9. Disconnect the hose from P manifold.
10. Lower into the water in order to keep end free for turning hose on 180°
D. FSO END STRING DISCONNECTION AND TURNING HOSE BY 180°

1. Mark the flanges of FSO end hose and Export manifold. This mark will be as a guideline
for Hose connection.
2. Prepare aft mooring winch with wire rope for lowering Export hose.
3. Disconnect belly wire and rope.
4. Disconnect 2 winker lights on hose.
5. Transfer weight of Export hose from turnbuckles to chain blocks which can be hooked
on the special manifold of the hose fitted with lifting lugs.
6. Disconnect Export hose from Export manifold and slack down little bit to make easy
install blank flange.
7. Fix 12” blank flange ANSI 150 connected with the fitting if required on FSO export hose
end.
8. Turn the hose by 180° by changing position of chain blocks
9. Connect hose to export manifold

C. TANKER END STRING

1. Pick up hose from water
2. Secure the hose by snubbing chain and keep the hose onboard and hang up with crane
same as when the hose position still connect with manifold (as per standard practice)
3. Full up hose with water
4. Fix 12” blank flange ANSI 150 connected with fittings for pressurize and depressurize
(included recorder and pressure gauge). Mark the flanges of the hose and valve. The
marks will be used as a guideline during the assembling.

fully completed and signed off prior to commencing this
procedure, as Appendix B at the end of this document.

3. Ensure there are no bridges or standing orders in place that

would make this task unsafe to continue. If a bridge or standing
order, does impact on safety do not continue with the job. Re-
plan the work when safe conditions will be met.

Order yang terpasang yang bisa mengakibatkan proses ini
tidak aman bila dilanjutkan. Bila ada, jangan teruskan
pekerjaan. Rencanakan kembali pekerjaan apabila keadaan
sudah aman.

 Fire monitors/Fire extinguisher.
 Safety shower/Eye wash station.
 ESD/PSD station
 Escape routes

5. Conduct a STEPBACK prior to commencing work, to check for
