Type of Clinical: Vision & Hearing screening for 85 children, grades 1-6
Age = 3-12 years

Nursing Diagnosis(es): Readiness for enhanced knowledge as evidenced by voicing
interest in obtaining information r/t to enhancing a healthy variety of veggies and
fruits.
Experience Gained: Communicating with school-age children and performing vision
and hearing screens. Engaging and interacting with a group of children from a diverse
background, with an interesting point of view. Developing my communication and
organizational skills looking after school age clients. practicing therapeutic
communication. Enhancing my skills conducting pediatric Vision & Hearing screening.

Nursing Diagnosis(es): Readiness for enhanced health management as evidenced by
expressed desire to enhance management of prescribed regimens r/t daily prescribed
medication in a timely manner.
Experience Gained: Using clinical judgment in reading blood pressure and answering
questions regarding blood pressure. Interacting with a minority group, that lacks
appropriate access to care. Providing clinical care in the community setting as well as
informing and advising clients on health promotion issues.

Medical Diagnosis(es): Acute pyelonephritis, UTI, renal obstruction, hypothyroidism ,
hyperlipidemia
Nursing Diagnosis(es): Readiness for enhanced nutrition as evidence by an expressed
willingness by client stating “I am willing to eat more if staff provided me with smaller
and more attainable portions of food during meals.
Experience Gained: Psychological and therapeutic communication and support
throughout the stay at the TCU, knowledge gained about the type of diagnosis and
medication the client was prescribed. Medication administration and a complete
health and physical assessment. Performing a comprehensive geriatric assessment.
Assessing the emotional response to the disability or limitation. Noting elimination
status such as usual pattern, present patterns, signs of constipation.

Medical Diagnosis(es): Urinary tract infection, major depressive order, single episode,
GOUT, Essential primary hypertension, paroxysmal atrial fibrillation, dementia,
gastro-ephageal reflex disease, begin prostatic hyperplasia, hyperlipidemia
Nursing Diagnosis(es): Readiness for enhanced mobility as evidenced by client
agreeing to ambulate and participate in an active range of motion with the presences
of a wife.
Experience Gained: Assisting a client with ambulation and transferring from bed to
wheelchair. Feeding a client, providing comfort through communication. Assess input
and output record and nutritional pattern. Monitor nutritional needs as they relate to
immobility. Checking for skin integrity for signs of redness and tissue ischemia. Giving
medications as appropriate. Turn and position the client every 2 hours or as needed.
