NURSING SERVICE GUIDELINES FOR AFFILIATES

1. Orientation is a must for all students coming to the hospital for the first time.
a. General Orientation
- Courtesy call to the Chief Nurse and the Chief of Hospital
- Lectures on:
a. History of Hospital
b. Functions of the Hospital
c. Sources of Fund
d. Basic Service to the Hospital and special projects
e. Organizational
Medical
Nursing
Administrative
b. Physical tour of the hospital by the respective C.I. with the Head Nurse in each dept.
2. Unit orientation to be given by the Head Nurses of the unit.
a. Ward policies
b. Routine
c. Staffing
d. Facilities
e. Patients
f. Demonstration on procedures which are special to the department and special apparatus.
3. Students reporting on duty must be on complete uniform as prescribed by the school and on
proper grooming. They must have their respective logbook for time-in and time-out at the
security guard post. C.I.’s time-in and time-out at the Supervisor’s Office.
4. Duty schedule of students must be provided by the C.I. in the units where the students assigned.
5. In the absence of C.I., students are not allowed to go on duty during 7-3 and 3-11 shifts while on
11-7 shift they can be absorbed by another C.I. or advised students not to report on duty.
6. During tour of duty, students are not allowed to use their cellphones except on emergency
cases. Taking pictures esp. during delivery is prohibited. Any loses like money, cell phone,
jewelries and other valuable things hospital is not liable.
7. The C.I. students, incoming, and outgoing, must participate in the endorsement.
8. Procedures to be done by students must be supervised by C.I., Head Nurse, Staff Nurse in DR,
OR and Nursery and all wards.
9. Students and C.I. are not allowed to make prescription, death certificate and to watch television.
10. No students are allowed to complete/get cases without the contract of affiliation duly
signed/approved.
11. No students can leave his/her duty without proper permission from C.I. or Head Nurses of the
unit so proper reliever could provide.
12. Students must consult their C.I. or Head Nurses/Staff Nurses for doubt on procedures to be
done.
13. Break time allowance for a) meal -30mins b) snack-15mins.
14. Students behavior:
a. Students must observe the following while on duty
- Silence
- Respect to staff and others
- Courtesy to patients and to public
- Passion in profession
- Heart to work
b. Drinking hard liquor and smoking are not allowed while on duty.
c. Loitering around is not allowed.
d. Sleeping/flat on the bed during hours of duty is not allowed.
15. Anecdotal report must be made by C.I., Head Nurse who witnessed the incident whenever
mistake is committed by the student. A copy will be provided to the Nursing Office.
16. Clinical experience for head nursing must be participated by:
a. Clinical Instructor-50%
b. Staff Nurse/Head Nurse -50%
17. The Daily Time Record of the C.I. must be signed by the Chief Nurse.
18. Don’t meddle regarding the hospital problems. Maintain harmonious relationship between staff
and C.I.
19. Absconded patients - students are not obliged to pay the bill of absconded patients but will
reflect on their performance rating. Never collect/solicit donations from students.
20. For students with car, use the right parking area.
21. Exposures of students at special areas esp. OR/DR/Nursery, ER, ICU, C.I. should supervise their
respective student during the said exposure.
22. Students are not allowed to stay in the Recovery Room, Dressing Room in OR and ER, Surgical
Clinic if no patients. Never stay in the secluded place and locked the door.
23. Never do procedures, injections, dressings without the supervision of your C.I. and Nursing Staff.
24. Use student conference room for 24 hours, but observe cleanliness and orderliness of the hall,
never leave your waste put it in a proper waste segregation
25. Orientation must be done in the morning and the office must be notified with a written request.
No orientation during 3-11 and 11-7 shift.
26. Requirements for Clinical Instructors must be:
a. IVT License
b. With a least MAN units
c. 1-2 years Hospital Experience
d. Must submit updated Resume
27. Preceptorship training of C.I’s should be 2 weeks in special areas and 5 days in each ward and
must submit all the requirements mentioned before the start of preceptorship training.
28. Students assigned in special areas such as OR, DR, and Nursery will bring glove for their own use.
29. Cases will be submitted by the school coordinator only with original proof attached; student will
not be entertained for individual submission of cases.
