Always maintain 20Km/h Speed while on site
SITE TRAFFIC REGULATIONS:
The site is a work area and as such, speed must
be controlled for all Equipment/Vehicles at all
times. The speed limit on site roads is 20Km/Hr.
PARKING:
There are dedicated Parking lots on site. Vehicles
must never be parked elsewhere, where they
block access / Fire hydrants. Where there are no
parking lots and a vehicle is parked, keys must
be left on the ignition so that they could be
removed during emergency. No vehicle must be
parked in a RESTRICTED AREA on site. In an
emergency, drivers must park on road shoulders
and move to the nearest muster point.
MODIFIED DRUMS OBJECTS:
Modified thinners drum used to collect garbage,
the modification i.e. particularly opened top
cover has created a serious laceration type
hazard.

PTW SYSTEM INSIDE BARRICADED AREA
Obtain necessary work permit issued by Q-chem
Operation (Q-Chem PTW System) and received
by Contractor/ subcontractor Lead Workers; All
supporting documents shall be available at site
prior to issue the permit, according to the
typology of the work to be done;
The Safety Plan for the Start Up operation shall
be followed; All personnel entering into the
barricaded areas shall wear the Nomex Coverall
(FRC); All personnel shall have a valid Hazcom
Card in order to be allowed to enter the
barricaded areas P-Pass card shall be attached at
gate’s P-Pass Board before entering the
barricaded area and shall be collected when
exiting from that area. All Managers and
Supervisor entering the barricaded areas need to
attend the Module # 44 Training and shows the
proper sticker in the helmet.
All sources of ignition are strictly forbidden inside
the barricaded area (i.e. mobile phones, lighters,
etc); only Intrinsically Safe radios are allowed
inside;
PTW SYSTEM OUTSIDE BARRICADED AREA
Until further notice, all work to be executed
outside of barricaded areas shall be done by
issuing a Contractor PTW, which shall be co-
signed by Q-Chem Operation Personnel. The
Permit Issuer & receiver must ensure full controls
are in place at the work site and communicate
clearly with the Co-signator, and any anomalies
found by either party, must be rectified prior to
the Permit being issued.
Contractor Area Safety Officer shall be contacted
by the Permit receiver or Issuer in order to track
all PTW issued in that area.
