IDENTIFYING DATA AND REASON FOR REFERRAL

PGL is a 19 years old who was referred for a psychological evaluation to determine her
current cognitive and emotional status. She is currently studying Bachelor of Science in
Industrial Psychology at EARIST Sampaloc, Manila.

SOURCES OF INFORMATION

Background of information was obtained from interview and observation during testing
and from standardized psychological test.

BEHAVIORAL OBSERVATION DURING TESTING

PGL is wearing her usual uniform, cream white blouse and maroon slacks with a leather
shoes.

Before she took the test, she has the complete understanding of nature and purpose of the
test. During the examination, she is listening attentively. She was motivated to perform the tasks
and failed to show signs of resistance, hyperactivity or distractibility. She tended to put her head
on the desk when task were monotomous or boring. She became engaged and more alert when
task challenges his interest and capability.

During her test in Personality Test that includes 16PF and Bell Adjustment Inventory, she
was observed of interested and high energy level. She said “Parang dito ko nalalaman kung
kilala ‘ko ba talaga sarili ko o hinde”.

GENERAL EVALUATION

During the test, PGL was generally showed pleasant and compliant attitude. There is
nothing that was observed which may affect her answering and completing the test. She was able
to respond to all tests.

In intellectual evaluation, almost all PGL’s test results were found to be within above
average. In Mental Ability Test, she obtained the rating of “above average” score 75th percentile.

On the SRA Verbal, she garnered “mid average” on the Linguistic part conveying that
she will probably have difficulty handling most verbal material. She also acquired an “above
average” on the Quantitative part denoting that she may be expected to adjust quickly and
accurately to situations involving numbers.

Meanwhile, she got 43 with 85 percentile interpreted as “above average” in Purdue Non-
Language Test. This signifies that PGL did quite well in terms of geometric figures and could
perform will in her academics.

In personality evaluation, PGL's 16PF test appears to have more low scores rather than
high score in assessment performed. These scores imply that she appears to be atruggling with
her emotion. She shows to be easily affected by her feelings. Although PGL got low score, she is
also seen to have high self-concept control.
 Based on Bell Adjustment Inventory test, PGL measured to have low score but also got
high score that shows her weaknesses. PGL, tend to be self-confident and assertive. Her results
also shows that she tend to be friendly and accepting other people. While, she tend to feel that
home relationship is unsatisfactory in terms of Home Adjustment. She also needs additional
attention in health adjustment.

RECOMMENDATION

According to the reviewed evidence, PGL is experiencing difficulties with the use of
language such as English and foreign language and mathematical problems. Further test should
be done to provide a better understanding of use of language and math processes. It is
recommended that PGL be given many different opportunities to practice using English
language. This includes reading books and practicing her communication skills. Regarding
mathematical, evaluation of her math tests from school is recommended to look for patterns in
her math abilities. It would be helpful to observe PGL solving math problems.

With regard to emotional support for PGL, further discussion with her are recommended to
see how she feels she is struggling with controlling her emotions and interpersonal situations.

 ANAMNESIS

I. Identifying Data

PGL is 19 years of age, female and living at Brgy. Silangan, San Mateo, Rizal. She is
single, birthday is on 6th June, 1998 and she was born in San Mateo, Rizal. PGL is presently
pursuing BS Industrial Psychology at EARIST Sampaloc, Manila.

II. Family History

PGL is the 3rd child of Marilou and Alvin. Marilou and Alvin are married and have a
close relationship with PGL. Marilou is a stay-at-home mother and Alvin works as a jeepney
driver. PGL has an older sister who is 22 years of age. She is married and currently living at
her husband’s residence. PGL has another sister who is freshly graduated and currently
looking for a job. PGL have also 2 younger siblings. PGL and her siblings are not totally
close with each other. There is no history of significant mental illness or cognitive deficits
PGL’s family.

III. Personal History

A. Prenatal

PGL has normal prenatal, natal and postnatal medical history.

B. Early Childhood (0-3 years old)

PGL was delivered normal by her mother. Her mother breast-feeds her until she
reaches 2 years old.
