INTRODUCCION
Los protocolos de Atención Inmediata y de Hospitalizados de los prematuros menor de 32 semanas y/o
menor de 1,500 grs. tienen como objetivo entregar herramientas que nos permitan enfrentar las situaciones clínicas
más frecuentes de una manera ordenada, disminuyendo la variabilidad clínica innecesaria y entregando una
atención segura a los pacientes que atendemos.
Los protocolos son recomendaciones para enfrentar las distintas situaciones clínicas. Si el documento
respectivo ha dejado algún aspecto que no se ajusta a la especificidad de la atención de un paciente determinado,
se espera que el o los profesionales involucrados en esa atención utilicen su criterio para decidir las conductas a
seguir. De esta manera el protocolo junto con entregar a los pacientes una atención más uniforme y segura, deja
siempre abierta las posibilidades de realizar los ajustes que cada paciente requiera. Así el profesional podrá
concentrar todos los recursos disponibles, sus conocimientos y criterio propio, su búsquedas en la literatura,
opiniones de pares y especialistas, etc., para la toma de decisiones respecto del requerimiento especial de su
paciente. Los requerimientos estándares ya están establecidos en estas recomendaciones.
Los protocolos no son en modo alguno un documento definitivo, sin embargo son documentos que forman parte del
programa de calidad de la unidad y se espera que se cumplan las recomendaciones que ellos contienen. Cuando
en la atención del paciente individual a juicio del equipo tratante determinada sugerencia del protocolo respectivo
no se aplica o no resuelve la situación clínica específica, esperamos que el profesional a cargo tome la decisión
que mejor se ajuste a la necesidad del paciente y que registre y avise por email a prematuro@neored.net de su
decisión. De esta manera se van perfeccionando las recomendaciones que contienen y desarrollando nuevas
consideraciones para que se ajusten mejor a las necesidades de la condición clínica que los protocolos abordan.
Los protocolos estarán disponibles para todos, médicos, enfermeras, matronas y técnicos en documento
escrito y en los computadores de la unidad. No se pretende que su contenido se aprenda de memoria, lo que
esperamos es que la consulta a este documento y el seguimiento de sus recomendaciones se incorpore a nuestra
cultura de trabajo.
Estamos seguros que estos protocolos iniciales más los que vendrán facilitaran nuestro trabajo clínico y
entregaran a los pacientes una atención clínica más segura y eficiente.

PROTOCOLO DE NUTRICION DEL RN PREMATURO < 32 SEMANAS Y/O < 1,500 grs.
OBJETIVO:
Estandarizar nutrición de los prematuros
1. Alimentación Parenteral:
AMINOÁCIDOS • 2 gr/kg/día
o RN < 1250 grs : Al nacer TODOS
o RN ≥ 1250 grs. : A las 72 hrs si se encuentra en régimen 0
ALPAR + Proteínas 3 gr/kg/día + Lípidos 3 gr/kg/día
LIPIDOS o RN < 1250 grs : A las 24 hrs TODOS
o RN ≥ 1250 grs : Al 5° día si se encuentra en régimen 0
Incrementar proteínas a 3,5 gr/kg/día a las 48 hrs. de iniciada ALPAR
Agregar vitaminas y oligoelementos al 5ª día de vida
Vía catéter venoso umbilical en RN ≤ 10 días. Vía percutáneo en RN > 10 días
Preparar ALPAR en 60 cc/kg/día con carga glucosa 5 mg/kg/min
Infundir lípidos en 20 a 24 hrs. Volumen infusión mínimo: 0,5 ml por hora
Preparar ALPAR para 48 hrs. Cambiar bolsa cada 48 hrs.
Si Dextro > 250 mg/dl, no suspender Alpar, usar insulina. Evaluar con coordinador
Suspender ALPAR cuando tolere 80 cc/kg de leche por al menos 24 hrs.
2. Alimentación Enteral:
RÉGIMEN 0 • RN < 1250 grs. o ≥ 1250 grs. PEG: 48 hrs.
• RN ≥ 1250 grs. AEG: 24 hrs.
LM o S S Care ® • Criterios para iniciar leche :
o sin vasoactivos, FiO2 < 0,40
o abdomen blando, residuo claro ≤ 2 cc, deposiciones ≥ 1 vez
VITAMINAS ACD • Iniciar al alcanzar un volumen de 150 ml/kg día por vía enteral
• Vitamina D: 400 UI/día
ACIDO FOLICO • Iniciar 50 microgramos por día al mes de vida
• Preparación ácido fólico 50 microgramos/ml (Total de 100 ml)

USO DE CUNA RADIANTE E INCUBADORA PREMATURO MENOR < 32 sem y/o < 1500 grs.
OBJETIVO
Estandarizar estabilización rápida de RN crítico en cuna radiante y un paso precoz de incubadora a cuna que
permita un apego y entrenamiento de padres prealta
PROCEDIMIENTO
INGRESAR A • Todo RN ≤ 32 semanas ó ≤ 1500 grs.
CUNA RADIANTE • Mantener sin ropa si cumple los siguientes criterios :
o Ventilación mecánica
o Catéteres umbilicales
o Catéter percutáneo

OBJETIVO:
Estandarizar el manejo de las apneas y los períodos de desaturación y bradicardia
CPAP NASAL
• 6 episodios en 6 hrs. que requirieron estimulación.
(PEEP: 5 cm H2O)
• > 1 episodio requiriendo ventilación a presión positiva en CPAP
CPAP CICLADO
(PEEP: 5 – 6 cmH2O, PIM: 18 – 20
• > 3 episodios en 6 horas que requieren estimulación
cm H2O, Frecuencia: 10 – 15
• 1 Apnea que requieren ventilación a presión positiva en CPAP
ciclos/min
ESTUDIO DE APNEAS
• ≥ 34 semanas edad corregida
CRITERIOS PARA ESTUDIAR • Apnea que requiere ventilación a presión positiva o conexión a
VM o CPAP nasal
ESTUDIO BASICO • Hemograma , PCR Hemocultivo (2),
• Punción lumbar histoquímico y cultivo de LCR según evaluación
tratante
• Radiografía de tórax AP
• Dextro, Sódio, Potasio, Cálcio, Gases venosos
• Panel (IF) viral en meses Mayo a Agosto
• Si Panel viral negativo y clinica compatible: PCR Bordetella pertussis
ESTUDIO SEGÚN CASO • TAC o RNM cerebro
• Electroencefalograma
• Screenning metabólico ampliado
• Ecocardiografía, Electrocardiograma
• Radiografía Esófago, estómago y duodeno y estudio de deglución

PROTOCOLO DE CIERRE DE DUCTUS RN PREMATURO < 32 sem y/o < 1500 grs.
OBJETIVO:
Estandarizar el manejo de ductus arterioso persistente
+ ECOCARDIOGRAFIA ALTERADA
• Relación diámetro aurícula izquierda/raíz de la aorta > 1,4 mm.
• Flujo holodiastólico reverso en aorta descendente
• Diámetro ductal > 1.5 - 2.0 mm/kg
• Relación tamaño de ductus/diámetro de la aorta descendente > 0,5
• Incremento del tamaño del ventrículo
TRATAMIENTO • Restricción Hídrica 90 - 100 cc/kg/día
• Mantener Hematocrito > 40%
• Furosemida 1 mg/kg e.v. c/12 hrs. por 3 días
• Ibuprofeno
+ ECOCARDIOGRAFIA ALTERADA
• Relación diámetro aurícula izquierda/raíz de la aorta > 1,4 mm.
• Flujo holodiastólico reverso en aorta descendente
• Diámetro ductal > 1.5 - 2.0 mm/kg

CRITERIOS DE ALTA DE RECIEN NACIDO EN RN PREMATURO < 32 sem y/o < 1500 grs.
OBJETIVO:
Estandarizar los criterios de alta de los prematuros
PROCEDIMIENTO
CRITERIOS DE ALTA • Edad Gestacional Corregida: ≥ 34 SEMANAS.
• Peso > 1500 grs. y alza de peso > 15gr/kg/día en 5 días previos al alta
• Termoregulando en cuna a 25 °C
• Alimentándose por chupete el 100 % del volumen indicado
• Sin apneas > 8 días
• Elixine suspendido > 1 semana
• Oxígeno suspendido en las últimas 72 hrs. con saturometría normal si
requirió oxígeno hasta 3 días previos al alta.
• En alta con oxígeno domiciliario: Ver Criterios de Alta Protocolo Displasia
Broncopulmonar
REQUISITOS PARA ALTA • Cumplir Criterio de alta
• Programa de educación realizado:
• Reanimación y atención de las emergencias pediátricas comunes
• Prevención muerte súbita
• Lactancia, extracción de leche materna (LM) y preparación de
mamaderas
• Cuidados del prematuro en el hogar
• Prevención de infecciones: Virus sincicial respiratorio (VRS), rotavirus,
vacunas del Programa ampliado de inmunizaciones (PAI)
• Protocolo de exámenes al día según protocolo de exámenes
• Ingreso a programa de seguimiento
PROGRAMA DE SEGUIMIENTO
IC FISIATRA o PREALTA > 34 semanas de edad gestacional corregida (EGC) en:
• Todo RNPT < 32 semanas o < 1,500 grs. al nacer
o Nuevo control según recomendación de Fisiatra
o Kinesioterapia motora y terapia ocupacional según indicación
Fisiatra
IC NEURÓLOGO o PREALTA > 34 semanas de edad gestacional corregida en:
• RNPT < 28 semanas o < 1,000 grs. al nacer
• RNPT < 32 semanas o < 1,500 grs. al nacer con HIC III -
IV, leucomalacia o convulsiones
o 40 SEMANAS de edad gestacional corregida
• TODOS
• Con ecografía cerebral o RNM según corresponda.
o Kinesioterapia motora y terapia ocupacional según indicación
médica
