Please note if you fail to attend this appointment without informing us, you will not be sent a further
appointment and will be discharged back to your GP.
You will only be able to reschedule your appointment on ONE occasion - If you cancel and reschedule an
appointment on more than ONE occasion you may also be discharged back to your GP. In these
circumstances you may be required to start your wait again.
Important note:
Please bring a list of medications that you are currently taking or have taken in the recent past either for your
skin or for any other condition you may have (from your GP, Pharmacy or alternative herbal remedies).
Please be advised that you may have tests or investigations performed on the day of your appointment. In
these circumstances you will be provided with further detailed information.

Opening times are: Monday to Friday 8 am to 8 pm and weekends and bank holidays 8 am to 4 pm (closed on
Christmas Day). Call charges from mobiles and landlines may vary. You will need your password to book,
cancel or change your appointment. If you do not have your password, please contact your referring practice
where a a member of the practice team can help. You do not need to see your GP.

Under the NHS Constitution, you have the right to start non-emergency, consultant-led treatment within 18
weeks from referral - or where cancer is suspected, to be seen by a specialist within two weeks of urgent
