Profile of Obstetrics and Gynecology Referral Case at DR Wahidin

ABSTRACT
Background: Health Service Referral System is a health service arrangement that
regulates the delegation of duties and responsibilities of health services on a
reciprocal basis either vertical or horizontal. The referral system is one of the
steps to improve accessibility, equity and improve the effectiveness of health
service and suited with patient needs including obstetric and gynecological cases.
DR Wahidin Sudirohusodo General Hospital is a Type A Hospital which is a
referral center for Eastern Indonesia.
Objective: To find out the general description of obstetric and gynecology referral
cases at DR Wahidin Sudirohusodo General Hospital. We also aim to evaluate
patient age, referral reasons, type of referral, and diagnosis.
Methods: This is a retrospective descriptive study of obstetric and gynecology
cases at DR Wahidin Sudirohusodo General Hospital. Data were collected from
digital medical records by 2017.
Results: In total from January - December 2017, referral cases based on age
showed the oldest age of referred patient is 25-29 years as many as 154 cases
(25.7%). Based on referral reason, the highest rate of referral case reason is
operation plan as many as 187 cases (30.4%), then treatment / ICU plan counted
162 cases (26.3%). The most referral type was APGO referral of 209 cases
(35.3%). Diagnosis with the highest rate of referral were severe preeclampsia in
89 cases (22.5%) and chemotherapy plans in 45 cases (11.4%).
Conclusion: The high rate of referral in severe preeclampsia becomes a particular
concern and requires further evaluation and research regarding the high incidence
of such cases in hospitals. This study has limitation in the completeness and
clarity of data in medical records so that it needs an evaluation in the process of
inputting and establishing medical record data to improve the quality of obstetric
health care system.
