Dear Admissions Committee:

I am writing a letter of recommendation on behalf of Eloisa Ann Legaspi

for an RN Nursing Program. I give Ms Legaspi my strongest letter of
support as I have known Eloisa for approximately 3 years and have had
the pleasure of working with her directly as her direct supervisor as the
UR Medical Director.

Eloisa covers Healthcare LA IPA, one of the largest medical groups in

the Greater Los Angeles area, with over 400,000 individual patients. She
has the immense task of following a large subset of the patients in this
medical group. Her main responsibilities include gathering clinical
reviews by conducting peer to peer discussions with case managers,
bedside nurses, and physicians. She reviews medical documentation
including physician progress notes, nursing notes, medication lists and
plans of care and helps coordinate a multidisciplinary team daily to
review high risk patients. Her role is to ensure that the patient’s care is
being optimally met and that safe discharge planning occurs. She follows
appropriate criteria and is well versed in the medical management of high
risk Medicaid patients. She additionally works with our high risk case
management team to ensure that readmissions are kept to a low number.
She truly cares about her patients and manages a large census with ease
given her persistent dedication to her field.

Some of the strongest qualities that Eloisa has is her empathy and desire
to provide the best patient care possible from a case manager perspective.
Eloisa has a strong clinical background as well, working in skilled
nursing homes. She is able to use her clinical background to enhance the
care of all the patients she cares for with our medical group. She has a
strong experience of clinical care and she applies this to her case
management duties and is a very valuable member of our team. She
encourages preventive care, patient education and patient empowerment.
I have seen directly how hard she works to take care of patients needs.
She is able to identify when a patient needs higher level of care In
addition, she promotes a culture centered around patient care and I give
her my strongest recommendation to pursue the RN. I have had the
pleasure of training medical students, fellows, residents and nurses at
LAC+USC Medical Center, and she has exceeded the expectations of not
only myself, but the executive leadership of our medical group.
