PATIENT TEACHING PLAN

Learner Assessment

Primary Health Problem: The patient has dementia, & Acute Encephalopathy. She is also a fall risk.
Learning Need: The patient needs to use the call bell and needs to understand the purpose of bed alarms
to prevent falls.
Readiness: The patient is alert and attentive. However, due to her dementia and acute encephalopathy,
she has limited cognition. She states, “I do not want to be here longer than necessary.”
Motivation: The patient is eager to leave the hospital and misses her family. She acknowledges she needs
help with standing & ambulating to decrease her risk for falling.
Learning Style: The patient stated she is a visual learner.
Reading Comprehension: The patient stated she likes to read romance novels.

Nursing Diagnosis
