Hamzah knows and understand his role as a student nurse and the unique role of other allied and medical staff that network to provide
safe and effective care for patients and their families. He adhere to the policies and procedures of the assigned agency, CNO and the
college standards. He ask relevant questions when he is not sure or when he needs to learn more.

Hamzah communicates and collaborates with the patient and families, nursing and allied staff to provide efficacious nursing care.Hamzah
research patient diagnosis and medication , collect pertinent data from kardex and patient chart on a weekly basis. He documented
medication administration, vital signs, and nursing interventions in a timely manner. Narrative notes are done in a timely manner. Hamzah
ensures patient privacy during nursing care and nursing interventions. He reported off to the nurse before taking breaks and gave patient
report to the upcoming nursing staff.

Hamzah had difficulty expressing himself initially, now he is more confident expressing himself. He build therapeutic and caring nurse
patient relationship and is able to disconnect from the relationship after learning the clinical setting. He provided client education before an
intervention or prior to medication administration.

Hamzah is very helpful to his peers with shower and bed making. He is willing to lend a hands when needed and gave feedback to his
peers. He always ask permission from patients before providing care to include his patient in the plan of care. He ensured his patients are
medicated, dressed , and is on time for their scheduled appointments.

H.T. : Each patient has different needs and should be thought of when making care plans. A stroke patient will need assistance due to the lack of

sensation on one side of the body.

Hamzah gather pertinent information from kardex, MAR, clinical notes, morning report and from his assessment of his patient to provide
and meet clients mutually agreed expected outcome. He provides documentation in a timely manner . He encourages his patient to ask for
help when needed and ensures call bell are in reach and the bed is in its lowest position. He performed bladder scan on his patient to
determined if he needed to perform catheterization.

6. COLLABORATIVE PRACTICE: INTERPROFESSIONAL HEALTH CARE TEAM

H.T. : consulting with OT's and PT's to see how much activity can be tolerated by the patient helps the nurse.

Hamzah communicated and collaborated with the nursing and allied teams to provide safe and effective nursing care. He followed through
with direction from allied team i.e. PT / OT to enhance patient newly learned techniques to achieve intended goals. That being said, he
ensures his patients do not miss any scheduled therapy appointments.

Hamzah has improved significantly in the clinical setting and towards, few week in the clinical setting he learned how to be self directed
and need minimal assistance from instructor to provide care for his patients. He seeks learning opportunities to enhance his clinical
performance i.e. bladder scan. He utilized theoretical knowledge and education materials to continue to provide safe and effective nursing
care. He followed the standards of CNO, BPG, and the polices and procedures of the assigned agency to improve his competence and
confidence in the clinical setting.
