Near the anniversary of each resident's occupancy, we carefully review the rent structure of the community in which
they make their home.
Just as day to day living expenses have gone up during the past year, so too have the expenses of operating rental
property. Therefore, in order to continue to provide healthy, well maintained communities, it becomes necessary to
implement a moderate rent increase.
The renewal rent for one year per month is: $1,425.00
The renewal rent for a two year lease:
*** Per month for the first year: $1,425.00
*** Per month for the second year: $1,495.00
PLEASE SELECT A RENEWAL OPTION BY COMPLETING AND SIGNING THE FOLLOWING PAGE,
AND RETURN BOTH PAGES ALONG WITH ALL RIDERS TO OUR OFFICE WITHIN THE NEXT
FIFTEEN (15) DAYS.
*IF VACATING AT THE END OF THE LEASE TERM, YOU MUST GIVE NO LESS
THAN THIRTY (30) DAYS WRITTEN NOTICE PRIOR TO THE EXPIRATION DATE
OF THE LEASE BY CERTIFIED MAIL RETURN RECEIPT REQUESTED. FAILURE
TO GIVE THIS NOTICE WILL RESULT IN FORFEITURE OF SECURITY DEPOSIT.*

LEASE EXTENSION
The lease which expires 04/28/18, between Robert Strauss, as tenant and Fairfield Bayview LLC as
Landlord in regards to Apartment 081 located at 234 River Avenue Apt 081, Patchogue, NY 11772 is
hereby extended under the following terms and conditions:
Except as modified herein, all terms and provisions of the original lease shall remain unchanged. This
renewal is not binding until countersigned by Landlord.
