2018 Arrowhead Little League All Star Form

It is time for us to look forward to selecting the players that will make up our All Star
Team, and represent Arrowhead Little League in District II/State and hopefully
beyond, and Presidents Cup. We recognize that there are many players who would
like to be on these teams. Players in the Major Division who are 11 or 12 years old,
and have played in at least 60% of the regular season games, are eligible for these All
Star Teams.
In order for your child to be eligible, you must understand and agree to the following conditions:

1) My child will be available to attend ALL practices and games, from June 1st through the
end of the competition, except as noted on page (2). I understand that failing to disclose known
dates of absence is cause for removal from the team. In accordance with Little League and Local
League policies, a player shall not participate in any other organized youth baseball from the date
they are named to the All Star roster (June 1st) until that team is eliminated from tournament play

2) I will provide an ORIGINAL BIRTH CERTIFICATE and 3 RESIDENCY

DOCUMENTS to Arrowhead Little League NO LATER THAN June 4th. Please note that other
forms of Birth Certification are not acceptable. If you anticipate your child being selected, and do
not have the original available, call now, and get a certified copy. It costs only $10, and is easily
obtained by calling the State.

3) I understand that the Minimum Play Requirement per game in All Stars is 1 At Bat and
players are not required to play defense. Each team is required to carry 12-13 players. While we
would like to play everyone as much as possible, there is no guarantee beyond the minimum.

4) I understand the cost of my child participating is an additional $70 to cover uniform and
tournament and post season costs and that such payment will be due at the time of verification on
June 4th.

If the 12 year old team continues to win, this team will ultimately go to the L.L. World Series.

12 year old All Star Team: DISTRICT II: June 20th Start date (Rio Vista Fields)
ARIZONA STATE: July 17th Start date (Scottsdale)
WESTERN REGIONAL: TBA (San Bernardino, CA)
L.L. WORLD SERIES: TBA (Williamsport, PA

11 year old All Star Team: DISTRICT II: June 18th Start Date (Rio Vista Fields)
ARIZONA STATE: July 12th Start Date (Gilbert)

11/12 year old Presidents Cup All Star Team: July 5th –July 10th ( Foothills Park)

and failing to disclose this information can be cause for removal from the team.
