AFFIDAVIT OF HEIRSHIP

I, MADELYN HABBILING AYASAO, of legal age and residing at #49 Honeymoon Road,
Baguio City, Philippines, after having been sworn in accordance with law, depose and say:

1. That I am the surviving spouse of the late PAULO FORAYOG AYASAO, an Overseas
Filipino Worker who died in Jeddah, Kingdom of Saudi Arabia;

2. That on April 30, 1999 we entered into a Contract of Marriage in Lagawe, Ifugao,
Philippines;

3. That on November 30, 2000, our first son MARK LOID HABBILING AYASAO was
born at the Ifugao State Hospital in Lagawe, Ifugao;

4. That on February 4, 2004, our second and last son LEO PAUL HABBILING AYASAO
was born at the Ifugao State Hospital in Lagawe, Ifugao;

5. That I attest to the fact that the following are the LEGAL HEIRS of the late PAULO
FORAYOG AYASAO:

MADELYN HABBILING AYASAO WIFE

MARK LOID HABBILING AYASAO, 9 years old SON
LEO PAUL HABBILING AYASAO, 6 years old SON

6. That as evidence of legal relation to the deceased, photocopies of our Marriage Contract,
Birth Certificate of Mark Loid Habbiling Ayasao, and Birth Certificate of Leo Paul
Habbiling Ayasao are herein attached; and

7. That I am executing this Affidavit of Heirship for myself and in behalf of my children,
who are minors, in relation to the Repatriation of the Remains of my late husband in
Jeddah, Kingdom of Saudi Arabia.
