This contract dated _____________________is made effective between ________________________________________

(“Parent or Guardian”) of _________________________________________________________________________(if player

is under 18) or __________________________________________________________________________________(“Player” if

18 or older) and the Wilkes Barre Scranton Knights. Parent and/or Player agree that player, if
selected, shall play for the Wilkes Barre Scranton Knights for the season beginning April 11, 2017
and ending April 10, 2018. Player is selected at the sole and absolute discretion of the Wilkes Barre
Scranton Knights coaching staff.

Parent and player agree to actively participate with the Wilkes Barre Scranton Knights and no other
USA Hockey or non-USA Hockey registered organization unless approved in writing or released by
the Wilkes Barre Scranton Knights. For the purpose of this contract, actively participating with the
Wilkes Barre Scranton Knights shall be defined as participation in scheduled games, practices and

showcases unless unable to do so due to illness or valid excuse with prior appropriate notice to the
Wilkes Barre Scranton Knights. Parent and Player agree that the Wilkes Barre Scranton Knights
shall have the right to release or suspend Player at the discretion of the Owner, General Manager of
Coaching Staff for any violations, on or off ice, requiring disciplinary action.

Parent and Player agree that for a player to remain on the team that the player must not commit
any act, on or off ice, that would be deemed detrimental to USA Hockey, the Eastern Hockey League
or the Wilkes Barre Scranton Knights, and must comply with all provisions of this stated contract
and remain a member in good standing.

Parent, where applicable, and player agree to hold harmless and indemnify the program, its officers,
volunteers, franchise holder, coaches and their respective representatives harmless from and
against any and all liabilities, damages, claims, or judgments which may be suffered by the Wilkes
Barre Scranton Knights by reason of any breach, negligence (gross or otherwise) or violations
arising from participation in the Wilkes Barre Scranton Knights activities and travels. Parent and
Player acknowledge that ice hockey can be a dangerous and violent sport in which to participate
and spectator, and accept and assume the risks associated with such participation therewith
including without limitation the risk of head injury, concussion and other serious bodily injuries as
well as the risks of travel to and from practices and games and being a spectator at the Wilkes Barre
Scranton Knights activities.

If Player is selected to participate with the Wilkes Barre Scranton Knights, Parent, and/or Player
agree to pay the Wilkes Barre Scranton Knights the following amount ($9,000.00) in the form of a
check made out to WBS Junior Hockey (5% discount for a one-time check and/or full payment
by September 1, 2017)

Any players who are not citizens of the United States will be responsible for paying their

All rostered players will need to submit the appropriate payment materials stated in this contract,
in order to keep their roster spot. There will be no exceptions to this policy.

Payment in the amount ($2,500.00), payable to the Wilkes Barre Scranton Knights must accompany
this signed contract to secure a potential spot on a roster. The following payment schedule will be
followed (Due By): TOTAL TUITION FEE = $9,000
 August 15, 2017 - $1,500.00
 September 15, 2017 - $1,750.00
 October 15, 2017 - $1,750.00
 November 15, 2017 - $1,500.00

Parent and Player agree and understand that all Eastern Hockey League tuition payments cover the
following:
1. Ice costs (practice and games)
2. Administrative and general expenses for operation of the team
3. Coaching fees to include a full time EHL Head Coach
4. Equipment & Outerwear: Helmets (NO facemasks), Gloves, Game Jerseys, Game Socks,
Shells, Equipment Bag, Practice Socks, Practice Jerseys
5. Apparel: Warm-Up Suits, Workout Shorts, Workout T-Shirt, Polo Shirt
6. Bus transportation for all trips that are 200 or more miles round trip. This includes all
league and non-league games and Showcases/tournaments
7. All costs related to game expenses
8. Eastern Hockey League Fees
9. All officiating expenses for regular season and playoff games
10. All hotel accommodations
11. Minimum of two meals per full day of game travel

12. Continuing educational resources (for post-graduates) and high school options for player
still in secondary school
13. Minimum of 4.0 hours of practice ice (when game schedule allows) per week
14. Off-ice Strength and Conditioning Program/Access to Fitness Facility

Parent and Player agree that if Player is selected for the Wilkes Barre Scranton Knights, all
other necessary forms will be executed, including but not limited to USA Hockey Individual
Registration, USA Hockey Waiver and Release of Liability form, USA Hockey Medical Release
and Consent to Treat form and appropriate Code of Conduct forms to be signed by BOTH

Parent and Player understand that the USA Hockey Annual Guide will determine any contractual,
property or financial issues in the event a player wants to separate from the Wilkes Barre Scranton
Knights after signing this contract. USA Hockey rules govern Junior Hockey and there are not
exceptions to the USA Hockey rules regarding player movement.

The Wilkes Barre Scranton Knights reserves the right to suspend or dismiss the Player from
the Team at any time for cause, including, but not limited to, unexcused absences from games,
practices, training sessions or other events, misconduct of a nature requiring disciplinary
action by the Wilkes Barre Scranton Knights or disruptive of unsportsmanlike behavior or
criminal activity or involvement.

Parent and Player understand that failure to meet any of the obligations set forth in this
Contract shall be grounds for suspension of Player’s participation in the activities of the
organization until such obligations have been fully met. In signing this Contract, Parent and
Player acknowledge that he or she has read the Contract, understands it fully: and willingly
accepts the financial and other obligations, which this Contract confers.

The tuition is non-refundable and no portion thereof will be reduced by reason of the Player’s
withdrawal, suspension or dismissal from the Team (except as governed by the USA Hockey Annual
Guide), injury or other incapacity or for any other reason whatsoever. By accepting a position with the
Wilkes Barre Scranton Knights, the parent(s)/guardian(s) and player understand and acknowledge
that there is a corresponding financial commitment on their part to the Wilkes Barre
Scranton Knights. ice time, coaching fees, gym membership, uniforms, etc. are paid by the Wilkes
Barre Scranton Knights. Tuition costs are calculated based on those costs and the number of
players who will be participating. By signing this contract, the parent(s)/guardian(s) and player
acknowledge that their child has filled a position that may not be able to be filled should the player
back out of their commitment. Therefore, failing to adhere to the payment schedule and/or
dropping out of the program after having signed the contract will not relieve the
parent(s)/guardian(s) of their financial commitment.
