AFFIDAVIT OF WAIVER AND QUITCLAIM

I, Shaine De Castro, Filipino, of legal age, single and a

resident of 222 Prinza St. Poblacion Muntinlupa City, after
having been sworn in accordance with law, hereby depose and
state that:

1. I am an employee of Levoglobin Pharmaceutical Inc. with

business address at Unit 1422 The RRCG Bldg., Filinvest
Alabang, Muntinlupa City.

2. I have filed my voluntary resignation with the Company

effective on June 30, 2018.

3. Upon proper computation by the Company’s Finance

Department, I am to receive the amount of One Hundred
Thousand Pesos (P1000,000.00) as final and full
settlement of all such salaries and employment benefits
in accordance with the pertinent employment contract
provisions and labor laws.

4. I hereby acknowledge receipt of the sum of One Hundred

Thousand Pesos (P1000,000.00), representing full
satisfaction of all my remaining salaries and employment
benefits and as a further manifestation of the
Levoglobin’s goodwill towards me and for my benefit.

5. In consideration of said payment, I do hereby quitclaim,

release, discharge and waive any and all actions of
whatever nature, expected, real or apparent, which I may
have against Levoglobin Pharmaceutical Inc., its
directors, officers, employees, agents and clients by
reason of or arising from my employment with the
company. I will institute no action, whether civil,
criminal, labor or administrative against Levoglobin
Pharmaceutical Inc., its directors, officers, employees,
agents and clients. The complaint for illegal dismissal
which I commenced before the Labor Arbiter against
Levoglobin Pharmaceutical Inc., are hereby deemed and
considered voluntary withdrawn by me and I will no
longer testify or continue to prosecute said action.

 6. I hereby also voluntarily, freely and unconditionally waive
any and all such claims, if any, and in executing this
waiver and quitclaim, I have fully renounce any claims or
right of action of whatever nature, whether past, present,
future or contingent against said corporation and its
officers, relative to my employer-employee relationship
with the company and the termination of such
relationship in view of my voluntary resignation from the
company.

7. I affirm that I was not forced into resignation and I

further affirm and confirm my resignation from the
company effective June 30, 2018.

8. I hereby confirm that this Waiver and Quitclaim is being

executed by me voluntarily and in a language familiar to
me and is not intended to circumvent or defeat any
provision of law, rule or regulation.

9. I hereby attest that I fully understood all the terms and

conditions of this affidavit as well as the legal
implications hereof and I hereby express my complete
agreement thereto.

IN WITNESS WHEREOF, I have hereunto set my hand

 BEFORE ME, a Notary Public, for and in the City of
