Project Proposal Template

1. Project Title

This should be descriptive and already encapsulates the general


objective of the project.

2. Project Proponent

Indicate name, address and other contact details of the proponent.


Detailed information about the proponent should be attached as
Annex A.

Annex A should contain the proponent’s profile including


background information and track record in implementing and
managing projects related or similar to the proposed project.
Describe the strengths and weaknesses of the project proponent.
How will the latter be addressed as it pertains to the proposed
project.

3. Project Background, Context and Justification (Maximum


number of words: 600)

Describe the situation prior to the project intervention and an


analysis of the situation that led to the proposed project. State and
elaborate on the social issues being addressed, the perceived
causes and consequences. What changes are expected as a result of
the proposed project? Such changes may include policy change,
capacity enhancements, delivery of services to the unreached
sectors and the like. Describe how the proposed project will change
and improve the current situation. How will the proposed project
contribute to development policies, plans or programs on national,
regional, LGU and community levels.

Outline reasons why this project is being proposed, what


issue/problem is being addressed through this project. While
justifying the proposed project it is essential to state the immediate
circumstances or conditions that led the proponent to initiate the
proposed actions or interventions.

4. Project Area (Maximum number of words: 400)

Describe the geographical boundaries of the project area and the


basis for selecting them as the project site. Describe the socio-
economic and environmental conditions of project area. Map should
be annexed showing location of project area within district or

provincial boundaries. State if the project area is being given or has
been given prior assistance by the project proponent.

5. Project Beneficiaries

Mention who and how many will be direct beneficiaries of the project
activities, e.g. a particular social group (children, farmers,
fishermen, women, etc.) or a particular natural resource (forest, fish,
livestock, wildlife, natural vegetation) As much as possible, indicate
the number and characteristics of direct beneficiaries. State the
number and characteristics of indirect beneficiaries as well.

6. Project Goal, Objectives, Expected Outputs and Outcomes

Describe the project goal and its SMART (Specific, Measurable,


Achievable, Realistic, Time bound) objectives, the deliverables or
immediate outputs of the proposed project. How will the output
contribute to the expected outcomes which refer to the longer term
results of the project.

It is mandatory to append a Logical Framework Matrix. See


Annex C for the format.

7. Implementing Strategy (Maximum word : 600)

Describe the overall approach and strategy of the project, the


project activities, roles and responsibilities of the project
stakeholders. If community is the direct beneficiary details should
be provided of envisaged community participation including gender
aspects. Will males and females participate in, contribute to and
benefit from the project?

Will there be partners in the project? Who are they and what roles
and contributions will they be expected to make? Describe how the
beneficiaries and stakeholders will participate in the design,
implementation and monitoring of the proposed project as deemed
necessary and appropriate. How will the project promote ownership
among the concerned stakeholders and how will such actions
contribute to project sustainability?

8. Project Success & Sustainability

What will be the success indicators of the project and what


measures will be taken to ensure its sustainability?

What are the risks to project success? How will these risks be
reduced or eliminated to ensure project success?

What assumptions are being made to ensure project success? State


and explain.

NOTE: The project executor will be encouraged to produce the
project’s results in prose form, at the end of project to popularize
the local initiatives in social development.

9. Monitoring Mechanism

Provide details about the internal monitoring mechanism of the


project i.e. who will internally monitor the project. Narrate when,
who and how the project monitoring will be done. What indicators
will be adopted, including means of verification and availability of
required data and information.

10. Project Cost

Please be reminded of the following funding restrictions of Children’s


Hour in coming up with the project cost, to wit:
a. Funding only covers direct project cost.
b. Depending on cost, funding can cover pediatric equipment
for differently-abled children, as well as classrooms.
c. Funding does not cover administrative costs such as
salaries for project managers, administrative personnel,
purchase of equipment, utilities, infrastructure and
contingencies.

For guidance in the budget breakdown, please refer to Annex D.

As well, Children’s Hour releases grants in tranches on a quarterly


basis. Funds are disbursed only on the following months: July;
October; January; and, April. Please refer to Annex E to show the
project cash flow for one (1) year.

Annex G: Documentary Requirements to be Submitted with


Project Proposal

1. Certificate of registration from SEC and other government and


certifying entities (certified true copy).
2. List of members of Board of Trustees/Directors and Key
Officers.
3. Board Secretary’s Certificate stating the Board Resolution
authorizing this project proposal submission.
4. Audited Financial Statement of at least the last fiscal year.
5. Certification of deposit from depository bank.

AGREEMENT: We affirm that the facts provided in this project


proposal are true and correct as of the date hereof. We hereby
authorize Children’s Hour to verify the information provided in this
application. We understand that if we are awarded funding, false
statements, omissions, or other misrepresentations made in this
proposal may result in the immediate withdrawal of Children’s Hour
from the Project.
