Re: Mr. Benjamin

I am writing to transfer Mr. Benjamin, a 63-year-old widower who is recovering from Total Hip
Replacement is about to be discharge today and needs continued care for his full recovery.

Mr. Benjamin was known to have an elevated blood pressure since 2003, which has had been controlled
by Ramipril 5mg. In 2005, he was diagnosed with osteoarthritis which required THR surgery. He was
then prescribed with aspirin 100mg.

During hospitalization, the delicate surgical procedure was a success. Upon normalization of his
sensation and successful three units blood transfusion, he has progressively started to walk with the aid
of a wheelie-walker.

Following discharge, Panadeine Forte 6 tablets per day is the pain reliever recommended to Mr.
Benjamin who also needs to do essential stretching exercises. The Care Well Hospital already provided
walker and wedge pillow. It would be greatly appreciated if you could facilitate hiring a toilet raiser from
a local medical supplier, this would help him transpire his mobility for two weeks. Also, kindly make sure
his dressing is changed every day. Lastly, please follow-up FBE and UEC tests.
