1. The existing online processing of requested student record/s is a joint project between the Treasury Department and the Office of the Registrar.
2. Online payment must first be credited by the Treasury Department through an online confirmation of the official receipt of complete payment. Otherwise,
such request shall remain under its pending status.
3. Please take note that request will only be processed upon the acknowledgment of Office of the Registrar with the official receipt number of the
requester.
For payment to be made from abroad, there is an additional cost of $10.00 for local bank charge. All other international bank charges are charged to the account
of the sender.

INSTRUCTIONS
STEP 1 : Online Application of Student Records
1. After Filling up the needed information for your online student records request, click "PRINT" to generate the PDF file of your Online Student Records
Application (OSRA) Form.
2. Save the generated PDF File, this file will be needed for the completion of the request.
3. Proceed to STEP 2.
STEP 2 : Payment
1. Pay the neccessary fee based on your OSRA Form within 7 days. Use the payment detail below for your reference.
2. After the payment is done, scan the copy of your deposit slip then proceed to STEP 3.
Note :If the requestee failed to pay the neccessary fee within 7 days, the Online Student Records Application the requestee made will automatically considered
as CANCELLED.
STEP 3: Email
1. Email the UST Office of the Registrar at registrar-online@ust.edu.ph and copy furnish treasury@ust.edu.ph, indicating the following :
1. Name of Applicant ( not the sender's or representatives name)
2. Payment made and purpose of remittance
2. Attach the following :
1. Scanned copy of the deposit slip
2. Valid ID of the owner of the Academic Records
3. Online Student Records Application (OSRA) Form
3. After emailing the UST Office of the Registrar,wait for the reply indicating the requestee's O.R. number. This will serve as your tracking number to
know the status of your online document request. Proceed to STEP 4.
1. Requester can track the status of his/her Online Student Records Application by accessing the website of the Office of the Registrar (
http://registrar.ust.edu.ph). Click "TRACK STATUS NOW" that can be found at the website homepage.
2. Provide the O.R. Number and Birthdate of the applicant to track the status of the requestee's Online Student Records Application.
3. When the status indicates 'FOR RELEASING', the requestee can now pick-up the document/s or wait for the document/s to be shipped(base on OSRA
Form).
Note : FOR PICK UP : If the requester will not be able to pick up the document/s personally, the representative must provide the following: written
authorization from the requester , valid ID of the requester and his/her representative.
The following instructions are part of your safety net. Everyone is enjoined to read and genuinely comply:
If the owner intends to send the requested student records applied for with the Office of the Registrar to a third party, he/she must send
1. An authorization letter specifying the complete details of the receiver (name, complete address, relationship with the recipient), duly signed by the
owner, bearing the specimen signature reflected on the scanned copy of a valid ID card, and
Reminders If the data are entered incorrectly and consequently lead to change of data, such request will be considered new request, hence, turn-around time
for the processing of document begins at Day 1. If requested document must first originate from the college, and then finalized in the Office of the Registrar, the
number of days for processing of such document (based on the website) would not be followed, thus, reasonable adjustment shall be necessarily observed.
Disclaimer
All student records that are requested for online processing but whose payment is not settled after 7 days, such requests are automatically cancelled.
