PETITION

Petitioner JOSEFINA ALMOJUELA PAULO, assisted by the Public

Attorney’s Office, thru the undersigned counsel and unto this Honorable
Court, most respectfully submits this petition for the correction of entry in
her MARRIAGE CONTRACT and avers as follows:

1. That petitioner JOSEFINA ALMOJUELA PAULO (JOSEFINA for

brevity) is of legal age, Filipino, and presently residing at Block 4 Lot
5 Dona-Ana Subdivision Camarin Caloocan City where she can be
served with summons, notices, orders, decision, and other processes
of this Honorable Court;

2. That respondent OFFICE OF LOCAL CIVIL REGISTRAR OF

MANILA is being sued in its official capacity and holding office at the
Manila City Hall where it can be served with summons, notices,
orders, decision, and other processes of this Honorable Court;

3. That respondent CIVIL REGISTRAR GENERAL and/or THE

PHILIPPINE STATITICS AUTHORITY is being sued in its official
capacity and holding office at the Solicarel Bldg., Ramon Magsaysay
Blvd., Sta. Mesa, Manila where it can be served with summons,
notices, orders, decision, and other processes of this Honorable
Court;

4. That respondent OFFICE OF THE SOLICITOR GENERAL is being

sued in its official capacity and holding office at 134 Amorsolo St.,
Legaspi Village, Makati City where it can be served with summons,
notices, orders, decision, and other processes of this Honorable
Court;

5. All other persons interested in this Petition are being impleaded in

accordance with Sec. 3, Rule 108 of the Rules of Court;

6. Petitioner JOSEFINA entered into Marriage with AURELIO

FRANCISCO PAULO JR. on 30 August 1980 in the City of Manila.
 The PSA Copy of their Marriage Contract is hereto attached as
(Annex A);

7. In the said Marriage Certificate, however, several erroneous entries

were recorded, to wit:

8. That the AGE of WIFE is “18” and not “21” as reflected in her
Certificate of Live Birth (Annex “B”),

9. That the BIRTHDATE of WIFE is “4/22/62” and not “4/22/59” as

reflected in her Certificate of Live Birth (Annex “B”),

10. That the LAST NAME of MOTHER of the WIFE is

“MATOCIÑOS” and not “MATACINAS” as reflected in her Certificate
of Live Birth (Annex “B”),

11. This Petition is done in good faith and is intended to set the
records straight and to prevent confusions in the future.

PRAYER
WHEREFORE, premises considered, it is respectfully prayed of this
Honorable Court that after notice, hearing, and publication, a judgment be
rendered ordering the Local Civil Registrar of Manila and the Philippine
Statistics Authority to effect the following changes in the MARRIAGE
CERTIFICATE OF AURELIO FRANCISCO PAULO JR. AND JOSEFINA
MATACINAS ALMOJUELA

TO CORRECT:

2. I have caused the preparation of the foregoing Petition;

the factual statements therein made are true and correct to the best
of my own personal knowledge and belief;

4. Further, I have not commenced any other action or proceedings

involving the same issues raised in this petition in the Supreme Court,
Court of Appeals, or any other tribunal or agency;

5. To the best of my knowledge, there is no case involving the same

parties and issues pending before the Supreme Court, Court of
Appeals or any other tribunal. Should I learn of the pendency of such
proceedings, I undertake to inform this Honorable Court within five (5)
days from such knowledge.
