AFFIDAVIT OF DESTRUCTION/MUTILATION OF PASSPORT

I, RACHEL P. ALVERO,o f l e g a l a g e , s i n g l e , F i l i p i n o c i t i z e n , a n d presently residing at

Brgy. San Isidro Jaro, Iloilo City, with after having been duly sworn to in accordance with law, do
hereby depose and say:

1. That I am a holder of Philippine Passport No. ER456728 valid until December 12, 2022;
2. That Last March 22, 2017, I noticed that the said passpost was damaged/mutilated due to
termite bites.
3. That the damage on the said passport has rendered i t u n u s a b l e f o r t r a v e l
purposes and therefore I request the Philippine Embassy/consulate to
issued a replacement passport in my favor;
4. That I attest to the veracity of the foregoing information.

JERELYN P. LIGARAY of legal age, single with postal address at Brgy. Quinar-upan,
Bingawan, Iloilo, hereinafter referred to as the DONOR,

along line 2-3 4 by lot 3-D both of Cad 25, Iloilo Cadastre; on the SE., along line 4-5
by lot 3-E of this subd survey; on the SW., along line 5-6 by 3272, on the NW.,along line
6-l by lot 3-E , both of CAD 25, Iloilo Cadastre .xxx Containing an area of Ten
Thouasand (10,000 ) square meters, more or less.
the DONEE is the younger sister of the DONOR, who dedicated her life to gain a
degree of Civil Engineering at Western Institute of Technology as a Com Laude for year 2018.

That for and in consideration of the DONNE’s dedication to her studies for a period of
five years, the DONOR hereby voluntarily GIVES, TRANSFERS, and CONVEYS by way of
donation, unto the said DONEE, his heirs and assigns, the above described property, together
with all the improvements found thereon, free from all liens and encumbrances;

That the DONOR affirms that this donation is not made with intent to deceive his
creditors, and that he has reserved for himself sufficient funds and property;

That the DONEE hereby accepts and receives this donation made in her favor by the
DONOR, and hereby manifests his gratefulness for the latter's generosity.
