PETITION

The undersigned petitioner, Alfons Janssen P. Marcera

, unto this Honorable Court respectfully states that: by
himself, to this Honorable Court, most respectfully alleges
that:

1. He is a Filipino citizen, married, born on January 4,

1986 and a resident of Bgry. Poblacion, Bacolod, Lanao del
Norte copies of the picture of the Petitioner with signature
at the bottom part are hereto attached as Annex “A”, “A-1”
and A-2”.

2. He has completed her law studies at the Mindanao

State University – College of Law Iligan Extension;

3. He has his office located at Purok 1, Poblacion,

Bacolod, Lanao del Norte with Telephone Number (063)
227-2109;

4. He was admitted to the Philippine Bar of July 31,

2017, per Attorney’s Roll No. 70427, a copy of the certificate
of admission is hereto attached as Annex “B”;

5. He is a member in good standing of the Integrated

Bar of the Philippines, Lanao del Norte Chapter and that he
is of good moral character as evidenced by the Certification
issued by two (2) executive officers of the IBP Lanao del
Norte Chapter, Atty. David Warren G. Lim, President and
Atty. Nestle S. Agopitac, Treasurer. Said Certification is
hereto attached as Annex “C”.

6. He has duly paid here IBP membership dues on July 18,

2017 as evidenced by IBP Official Receipt 011973. The said
Official Receipt is hereto attached as Annex “D”;

7. He has never been convicted of a crime involving

moral turpitude as evidenced by his clearance from the
Regional Trial Court, hereto attached as Annex “E” and
clearance issue by the National Bureau of Investigation,
hereto attached as Annex “F”;

8. He has no pending administrative case as evidenced

by the certification issued by the Office of the Bar Confidant
which is hereto attached as Annex “G”;

9. He has paid her Professional Tax for the fiscal year

2017 under Profession Tax Receipt 3255261, a copy hereto
attached as Annex “H”;

10. He has attached herewith and made integral parts

hereof his specimen signatures and true impressions of his
notarial seal as Annex “I to I-3”;

11. He has paid the filing fee as evidenced by Official

Receipts No. 4638476C, UPLC 1188175 and 4636138C
issued by the Office of the Clerk of Court, Regional Trial
Court of Lanao del Norte at Iligan City which are attached
hereto as Annex “J, J-1 and J-2”;

12. He possesses all the qualifications and none of the

disqualifications of a Notary Public;

13. He has not applied for commission before any

other jurisdiction;

14. He undertakes to inform the Executive judge

within ten (10) days in case of any change in the given
address and to submit his monthly report religiously;

15. He now desires to be granted Notarial Commission

as Notary Public for and in the City of Iligan and in the
Province of Lanao del Norte.
 PRAYER

WHEREFORE, in view of the foregoing, it is

respectfully prayed of this Honorable Court, after due
notice, publication and hearing to grant this Petition for
appointment of Notarial Commission of the herein Petitoner
for and in the City of Iligan and in the Province of Lanao del
Norte.

I, Alfons Janssen P. Marcera, of legal age, Filipino,

and a resident of Poblacion, Bacolod, Lanao del Norte, after
having been sworn to in accordance with law, hereby
depose and say that I am the petitioner in the above-entitled
petition, that I have caused the preparation of this petition
and has read the same and that all factual allegations
therein are true of my own personal knowledge and the
documents are based on authentic records.

That I further certify that: I have not theretofore

commenced any other action or proceeding or filed any
claim involving the same issues or matter in any court,
tribunal, or quasi-judicial agency and, to the best of my
knowledge, no such action or proceeding is pending
therein; if I should thereafter learn that the same or similar
action or proceeding has been filed or is pending before the
Supreme Court, the Court of Appeals, or any other tribunal
or quasi-judicial agency, I undertake to report such fact
within five (5) days therefrom to the court or agency wherein
the original pleading and sworn certification contemplated
herein have been filed.
