II. OFFER.

The testimony of the witness Michael Mano is being offered to prove the:

1. To prove the authenticity of the Medical Certificate from Makati General

hospital;

III. JUDICIAL AFFIDAVIT PROPER.

I, Roy Melchor 35years old, married, Filipino, and residing at no. 234 Burham ST.
Barangay Forbes, Makati City, under oath, depose:

Q – Please state your name, age, residence, and occupation of the witness.

A – I am Roy Melchor , 35 years old, married, residing no. 234 Burham

ST. Barangay Forbes, Makati City, and a doctor at the Makati General Hospital.

Q- Why are you here now?

A – To give a sworn statement by way of a judicial affidavit, the same to constitute

as my direct testimony, in the above-captioned civil case.

Q- For the record, please state the name and address of the Lawyer who is
now conducting or supervising your examination and the place where the
examination is being held now?
A – The legal counsel for the plaintiffs, Atty. Raposon Neil Patrick. is conducting or
supervising my examination now at his office at Makati CITY PROSECUTOR’S
OFFICE located at: J.P Rizal, Makati City

Q – In what language do you want your examination to be conducted?

A– This judicial affidavit is prepared in English.

Q – Do you undertake to answer the questions to be asked of you, fully conscious

that you will do so under oath, and that you may face criminal liability for false
testimony or perjury?

A -Yes.

6. Q – Let us now proceed to the Complaint. Do you know the complainant

this case?

A – Yes. he is one of the patient who went to the hospital for his injuries in his face.

7. Q – Can you describe what happen on the day the complainant went to
hospital?

A - Yes Sir. The complainant was suffering from a few bruising especially in his
mouth area. He was bleeding and his face was swelling due to the injury he
sustained.

8. Q- How does one get those injuries?

A – One could get those injuries by fighting or by being attacked by another

person.

9. Q- After you treated the complainant what else did you do?

A – I advice the complainant to get some rest for a couple of days until the swelling
in his face stop. He then ask me if he could have a Medical Certificate for his injury
so I wrote him one detailing the extent of his injuries.

10. Q - I offer to the court the Medical Certificate (Annex C.1) given to the
complainant. Who is the undersigned on this certificate?

A - It is mine. I signed the Medical Certificate for the complainant.
