DBES Student/Parent Guidelines/Contract

for Assessment Retake 2018-19

Grades 2 – 5
1. Students can demonstrate mastery of assessments when parents provide at home study
sessions and the student retakes the test (making corrections on missed items) during class.

2, Any student has one opportunity to make corrections on any end of unit assessment within
one week following notification of the grade. This will be available to any student who wishes to
improve their grade.

3. Contracts will require a minimum of two at home study sessions. This will be documented by
a checklist and parent signature on the contract. Students must return the original test with a
signature, and the contract with a signature, prior to the retest.

4. Half credit will awarded to the final grade for students who have demonstrated mastery by
making corrections. For example, if a student has a 90, then the highest grade that can be earned
when all items are corrected is a 95.

5. Students will make the corrections in the presence of the teacher.

Failure to meet EACH and EVERY criterion listed below will disqualify the student from the
opportunity to retake the test.

Students:
1. Meet with YOUR teacher to let them know you wish to retake the test. Agree on a date and
time with the teacher that you will have the retake completed. Obtain the Test Retake Contract
from your parent or from the teacher. Complete two at home study sessions for a total of at least
45 minutes. Use notes, text, online resources, etc.

2. Have both the contract and your first attempt at the test signed by your parent(s) and return it
to your subject area teacher.

3. On the day of the retest, your teacher will provide you with space and time to retake the test.

4. This test MAY or MAY NOT be the same one you took originally, but will cover the same
content. You will only be asked to correct questions you missed.
